Leadership & Staff Development:
Collaborate with nursing leadership to provide clear direction and support to nursing staff. Uphold Redstone's core values, address performance issues, and maintain open communication with all employees. Clinical Coordination:
Actively participate in daily interdisciplinary team meetings to ensure seamless care coordination. Support accurate and comprehensive documentation of resident conditions and ensure timely communication with physicians and families regarding changes in status. Resident Care Management:
Coordinate all aspects of resident admissions, care conferences, hospital transfers, and discharges. Attend and actively contribute to care conference meetings. Physician & Provider Liaison:
Assist with processing physician orders and participate in rounds with physicians and other supportive services. Quality Assurance & Compliance:
Review outstanding documentation and tasks to ensure completion. Serve as the primary contact for the contracted wound care group, coordinating weekly rounds and ongoing collaboration. Participate in Quality Assurance and Infection Control committees and assist with the development and implementation of corrective action plans for regulatory agencies. Staff Education:
Assist in developing and implementing staff education programs through huddles, one-on-one sessions, and virtual training. Patient & Family Advocacy:
Intervene in suspected abuse situations and assist in resident investigations. Serve as a resource for families, addressing concerns promptly and providing exceptional customer service. Team Support:
Provide hands-on support for medication passes during staff shortages or vacancies. Foster a culture of inclusivity and teamwork across the department. Qualifications:
Active Registered Nurse (RN) license in the state of Pennsylvania. Experience working in a long-term care or skilled nursing facility. Strong knowledge of nursing principles, practices, and procedures. Excellent leadership, communication, and interpersonal skills. Proficiency in electronic health records (EHR) and documentation systems. Commitment to providing compassionate, high-quality care. Ability to work effectively in a fast-paced, interdisciplinary team environment.
