Summary
Under the direct supervision of the Director, the Clinical Case Manager provides professional, compassionate, and evidence-based care within Alcott's Housing Support Services (HSSP) Program. This position serves adults residing in project-based housing with a full spectrum of DSM diagnoses. Responsibilities include conducting assessments, developing treatment plans, facilitating psychosocial intakes, providing crisis intervention, and delivering rehabilitative services as part of a multi-disciplinary team.
Registered associates may obtain BBS hours while receiving training, supervision, salary, and employer-paid benefits.
Salary Range:
$70,000 - $75,600 annually, DOE
Key Responsibilities
Assessment and Treatment Conduct psychosocial assessments and develop individualized treatment plans. Facilitate intakes and ongoing evaluation of client needs. Provide individual counseling, case management, and rehabilitative interventions. Therapeutic Services
Deliver evidence-based, trauma-informed interventions. Provide crisis intervention and stabilize urgent client needs. Support recovery, relapse prevention, and community integration. Care Coordination and Linkage
Connect clients to medical, mental health, and substance use services. Link clients to housing, social services, and community-based resources. Provide field-based support in client residences and community settings. Collaboration and Documentation
Maintain accurate, timely documentation in compliance with LAC-DMH, Medi-Cal, and agency standards. Participate in case conferences, staff meetings, and multi-disciplinary team collaboration. Consistently meet agency productivity and documentation standards. Other Duties
Perform additional responsibilities as assigned to support program operations. Qualifications
Required
Master's degree in social work, Marriage and Family Therapy, Counseling, or related field. Current California BBS registration (ASW, AMFT, APCC) or licensure (LCSW, LMFT, LPCC). Valid California Driver's License, clean driving record, and insurability under agency policy. Live Scan clearance and eligibility to bill Medi-Cal. Preferred
Experience providing services in community mental health, housing support, or residential settings. Knowledge of LAC-DMH documentation requirements. Bilingual in Spanish and English. Work Schedule & Setting
Full-Time, 40 hours per week Monday - Friday, 8:30 a.m. - 5:00 p.m. Based in Culver City, CA with field-based responsibilities
