Job Summary
The Community Health Worker (CHW ) serves as a trusted liaison between healthcare providers and the community. They promote wellness, facilitate access to services, and empower individuals to take control of their health. CHWs are often members of the communities they serve, which enables them to build trust and deliver culturally appropriate care and education.
Temporary Employment Services (TES ), a unit of the Auburn University Department of Human Resources, is an in-house support center established to meet the temporary employment needs of the university.TES provides qualified and dedicated temporary employees in a wide variety of occupations to meet the staffing needs throughout the campus. Temporary employees are hired for a variety of reasons with the most common being:
Assistance in the place of a regular employee who is absent for a specified period of time
Additional assistance during periods of abnormal or peak workloads
Assistance with special projects
Seasonal work
Emergencies

Essential Functions
Conduct home visits to educate individuals and families on health practices and available resources.
Assist patients in managing chronic conditions and navigating healthcare systems.
Provide health education and wellness coaching.
Refer individuals to appropriate healthcare providers and social services.
Lead workshops, support groups, and seminars on health-related topics.
Advocate for community health needs and help reduce barriers to care.
Collect and report data to inform healthcare providers and improve services.
Organize and participate in health screenings, immunization clinics, and outreach events.
Maintain accurate records and ensure patient confidentiality.
Collaborate with healthcare professionals, local agencies, and community organizations.

Minimum Qualifications
High school diploma or equivalent
Valid driver's license and reliable transportation
Expectations
:
Fluent in English
Experience in community outreach or health education
Desired Qualifications
CPR and First Aid certification
Bilingual (Spanish)
Certification as aCHW (training will be made available if not already trained)
