Job Summary:
The Medication Reconciliation Pharmacy Technician plays a critical role in ensuring patient safety by obtaining and documenting accurate medication histories (including medication allergies and intolerances) for patients upon admission, transfer, and discharge. This technician collaborates closely with pharmacists, nurses, and physicians to support optimal medication management and reduce medication-related errors. Essential Functions: Consistently uses an outward mindset and puts forth exemplary effort in accomplishing his/her goals and objectives in a manner that helps others to achieve their goals and objectives. Conducts structured patient interviews to obtain comprehensive medication histories, including prescription, over-the-counter, herbal, and supplement use. Contacts outpatient pharmacies, long-term care facilities, and primary care providers to verify medication lists. Documents medication histories, including medication allergies/intolerances, accurately in the electronic health record (EHR) system. Identifies discrepancies between home medications and hospital orders and escalate to pharmacists for resolution. Assists in the reconciliation process during patient transitions of care (admission, transfer, discharge). Maintains confidentiality and comply with HIPAA regulations. Participates in quality improvement initiatives related to medication reconciliation. Supports training and onboarding of new technicians or pharmacy students as needed. Medication reconciliation in other locations ED unit inspections, crash cart inspections, pyxis machine outdates and ED pyxis machine minor repair. Minimum Qualifications:
Active Michigan Pharmacy Technician license. Certified Pharmacy Technician (CPhT) through PTCB or NHA. Minimum one (1) year of experience in a hospital or clinical pharmacy setting. Knowledge, Skills & Abilities:
Experience with medication reconciliation or patient interviewing preferred. Familiarity with HER systems, preferred. Bilingual abilities preferred. Ability to work independently and as part of a multidisciplinary team. Strong communication and interpersonal skills. Able to work accurately under stress and with time interpersonal skills to effectively communicate with personnel and visitors. Able to operate computer programs, troubleshoot, and resolve system issues; must have strong organizational skills. Working Conditions:
May be exposed to infectious waste, diseases, conditions, etc., including the AIDS and Hepatitis B viruses . Must wear appropriate personal protective equipment as required in sterile areas. Physical Requirements
