No Job Summary: Conducts health assessment interviews focused on wound, ostomy, and continence care. Performs focused physical assessments, develops individualized plans of care, and recommends medications and treatments as appropriate. Job Responsibilities: Develop, revise, and evaluate standards of care; assist in selection of supplies, technology, and equipment. Assess and document patient responses to wound, ostomy, continence, fistula, and complex wound treatments. Evaluate the quality of care provided by clinical staff. Collaborate with the healthcare team and make referrals to other disciplines as needed. Teach, counsel, and assist patients and families in preventing complications related to wounds, ostomies, or continence problems. Develop, provide, and evaluate staff education programs, including new employee orientation, in-services, and continuing education. Conduct comprehensive assessments of patients at risk for pressure injuries and initiate preventive measures. Escalate and communicate challenges, including ethical issues and conflict resolution concerns. Collaborate with the Assistant Director of Nursing for Wound Care to recommend best practices based on assessment. Participate in Unit/Department and Hospital-wide Quality Improvement initiatives, including incidence and prevalence studies. Provide culturally competent education to patients, families, and staff. Maintain security and confidentiality of patient information in accordance with hospital policies. Maintain professional competency through engagement in clinical practice and continuing education. Identify and refer patients to appropriate community resources. Manage assets related to rented equipment, e.g., specialty beds and negative pressure devices. Required Skills & Experience: Minimum of
