We are seeking an experienced Registered Nurse Case Manager to join Providence Holy Cross Medical Center. The ideal candidate will have a minimum of 2 years acute care hospital case management experience, with demonstrated skills in care coordination, discharge planning, and utilization of InterQual criteria. This position requires travel experience (first-time travelers may be considered) and offers an opportunity to work in a dynamic hospital setting. Key Responsibilities
Perform care coordination and discharge planning for acute care hospital patients. Assess patients’ clinical status and participate in family meetings. Develop and contribute to individualized treatment plans. Utilize InterQual Admission Criteria for patient admissions (Emergency Department focus). Arrange Durable Medical Equipment (DME), Skilled Nursing Facility (SNF), and ambulance transportation independently. Collaborate with interdisciplinary teams including social services and transportation. Manage patients with complex needs, including those with drug/alcohol abuse, homelessness, mental health history, or crisis situations. Maintain computerized documentation and charting using Epic system. Participate in regulatory compliance activities including CMS and DRG criteria adherence. Qualifications
Registered Nurse with current licensure. Minimum 2 years acute care hospital case management experience required (behavioral health-only experience is not acceptable). Experience with care coordination, discharge planning, and utilization review. Proficiency with Epic electronic health record system. Familiarity with HMOs, IPAs, and hospital regulatory standards. Ability to organize and conduct family meetings and treatment plan development. Competent in arranging patient transportation and DME. Strong organizational and communication skills.
