Job Description
We are seeking an experienced Acute Care Hospital Case Manager with a strong background in care coordination and discharge planning. The ideal candidate will possess the necessary skills to effectively manage patient care and facilitate a smooth transition from hospital to home or other care settings.
Responsibilities
Perform care coordination and discharge planning for patients in acute care settings. Utilize InterQual Criteria for assessing patients on admission. Engage in family meetings and develop patient treatment plans. Arrange Durable Medical Equipment (DME), Skilled Nursing Facility (SNF) placements, and ambulance transports as needed. Provide support for patients dealing with drug/alcohol abuse, mental health issues, and homelessness. Required Experience / Certifications / Licensure
Minimum of 2 years of acute care hospital case management experience. Experience with care coordination and discharge planning is essential. Proficient in the use of Epic charting system. Knowledge of various patient demographics including adolescents, adults, and geriatrics. Experience working with HMOs and IPAs is preferred. Why ARMStaffing?
