Patient Care Coordination Associate

plays a vital role in our team by managing all aspects of scheduling new patient evaluations and follow-up appointments, answering phone inquiries, and optimizing routes for home visits. This position does not involve direct patient interaction. Location : 6545 Corporate Centre Blvd Suite 240, Orlando FL 32822 Schedule : Monday-Friday: 8am-5pm. Primary Responsibilities: Conduct telephonic outreach to patients and enhance their experience through calls. Coordinate patient care per instructions from physicians, adhering to company standards and policies. Support and follow the Standard Delegation of Orders (SDO). Schedule evaluations and follow-up appointments for Palliative Care providers. Map driving routes for in-home visits using web-based applications like Google Maps. Build referrals in Electronic Medical Record (EMR) systems. Maintain an up-to-date master list of Palliative/Bridges Patients for reporting purposes. Keep SharePoint updated with all patient information changes. Answer phone calls and resolve inquiries or route them accordingly. Manage medical records including filing, scanning, and preparing for scheduling. Ensure timely scanning and filing of all correspondence. Process requests for medical record releases, maintaining appropriate logs. Work independently and perform all related duties as assigned. Required Qualifications: High School Diploma/GED or higher. Over 2 years of customer service experience, including data entry and effective communication skills. At least 1 year of working knowledge of medical office procedures and terminology. Experience using Internet navigation tools like Google Maps. Advanced proficiency in Microsoft Office applications, including Excel, Word, and Outlook. Preferred Qualifications: Experience in a physician's office, clinical, or hospital setting. Familiarity with EMR systems and/or TruCare. Prior experience in a call center environment. Bilingual proficiency in English and Spanish. The salary range for this role is $16.00 to $28.85 per hour, based on full-time employment. Compensation may vary depending on local labor markets, education, and experience.
