Under the Department of Pharmaceutical Services, the Clinical Staff Pharmacist II provides pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System. They may also participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed. The Clinical Staff Pharmacist II performs a wide range of pharmaceutical care functions to ensure drugs prescribed for and used by patients are safe and effective. This includes obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on proper use of medications, and ensuring patients understand cautions, adverse reactions, and interactions related to their therapy. The Clinical Staff Pharmacist II works as part of an interprofessional team with physicians, advanced practice providers, nurses, and other professionals. This role is based primarily in the inpatient pharmacy setting, with potential to oversee pharmacy technicians to facilitate insurance authorizations and to prepare and dispense prescription medications and pharmaceutical products for patient care. The Clinical Staff Pharmacist II may also be responsible for oversight of clinical activities and delivery of services. Location: Inpatient/acute care settings; Schedule and site: dependent on operational needs. Responsibilities

Provide pharmaceutical care to patients at UCSF Health System acute care pharmacy locations; obtain medication histories; review and assess medication profiles; provide drug information to patients and health care providers; collaborate with clinicians on drug choices and therapeutic plans; ensure access to medications; educate patients on the proper use of medications; explain cautions, adverse reactions, and interactions related to therapy. Be part of an interprofessional team (physicians, advanced practice providers, nurses, and other professionals). Based primarily in the inpatient pharmacy setting, manage a specific patient population if applicable. May oversee pharmacy technicians to facilitate insurance authorizations for medications; prepare and dispense prescription medications and pharmaceutical products for patient care. May be responsible for oversight of clinical activities and delivery of services. This position requires flexibility to orient and work at all UCSF Health locations depending on operational needs. Qualifications

Required Qualifications

Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy Preferred Qualifications

Completion of an ASHP-accredited PGY1 Pharmacy Residency, or at least 3 years of inpatient/acute care services pharmacy experience Hold an advanced practice board certification (e.g., Board of Pharmacy Specialty certificates, AAHIVP, etc.) Experience participating in pediatric and adult emergency situations Licensure/Certification

Must possess a current license in good standing to practice pharmacy in the State of California Current certification for Basic Life Support (BLS) from the American Heart Association Salary and Benefits

The salary range for this position is

$89.98 - $99.50

(Hourly Rate). The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on factors including work experience and internal equity within this position classification at UCSF. For positions represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
