Pharmacist – Infusion Full Time • Location: San Francisco, CA • 86249BR Job Summary: Under the Department of Pharmaceutical Services, the Clinical Staff Pharmacist II provides pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System. They may participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed. The Clinical Staff Pharmacist II performs a wide range of pharmaceutical care functions to ensure that drugs prescribed for and used by patients are safe and effective. These include obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on proper use of medications, and ensuring that patients understand cautions, adverse reactions, and interactions related to their therapy. The role will be part of an interprofessional team and based primarily in the inpatient pharmacy setting, managing a specific patient population if applicable. Depending on the assigned shift and site, the Pharmacist may oversee pharmacy technicians to facilitate insurance authorizations for medications, as well as prepare and dispense prescription medications and pharmaceutical products for patient care. The Pharmacist may also be responsible for the oversight of clinical activities and delivery of services. This position requires flexibility to orient and work at all UCSF Health locations depending on operational needs, as applicable. The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on factors including work experience and internal equity within this position classification at UCSF. For positions represented by a labor union, placement within the salary range will be guided by the collective bargaining agreement. The salary range for this position is $89.98 - $99.50 (Hourly Rate). The Department of Pharmaceutical Services provides a full range of services for patients cared for by the Medical Center at UCSF and its providers and payors. These services include drug procurement, storage, and security; safe, accurate and timely preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies; drug information services for clinicians and patients; and participation in technology assessment and quality assurance programs. The Department is affiliated with the UCSF Department of Clinical Pharmacy, which has primary responsibility for teaching professional courses in the Pharm.D. curriculum, clinical research, and public and clinical service. Required Qualifications

Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy Preferred Qualifications

Completion of an ASHP-accredited PGY1 Pharmacy Residency, or at least 3 years of inpatient/acute care services pharmacy experience Hold an advanced practice board certification (e.g., Board of Pharmacy Specialty certificates, AAHIVP, etc.) Experience participating in pediatric and adult emergency situations License/Certification

Must possess a current license in good standing to practice pharmacy in the State of California Current certification for Basic Life Support (BLS) from the American Heart Association About UCSF
