Overview
Home Health Registered Nurse (RN)
at
– a role delivering expert care in patients’ homes, where healing happens best. You will work independently and as part of an interdisciplinary team to conduct comprehensive assessments, develop and implement individualized care plans, and deliver outcome-driven interventions under the physician-ordered Plan of Care. This position reflects the principles of community health nursing and a commitment to patient- and family-centered care.
Position Responsibilities
Possess knowledge of developmental needs and age-related considerations for the patient population served.
Conduct and document a comprehensive patient assessment upon admission.
Continually reassess and apply advocacy and interventions as indicated.
Interpret patient self-assessment and behavior with consideration of cognitive, physical, emotional, psychosocial, and maturation factors.
Collaborate with patient, family, and healthcare team to develop, implement, and monitor an individualized plan of care based on best practices and evidence-based medicine.
Adapt to changes and updated patient care practices within an evolving healthcare environment.
Document patient care activities accurately, concisely, and timely in medical records (paper and electronic).
Assist with orientation of new staff and students as needed.
Acquire necessary training to competently perform preceptor and charge nurse functions when selected.
Advance professional nursing practice through continuing education, required training, and participation in department activities.
Foster open communication and a patient- and family-centered care environment; maintain professional relationships with the care team.
Demonstrate adaptive and timely communication based on individual patient/family needs.
Attend or read minutes of at least 50% of scheduled staff meetings and stay informed on content.
Education And/or Experience
Required:
Associate or Bachelor's degree from an accredited School of Nursing.
Preferred:
1 year of recent experience in home health, acute care, or skilled nursing facility.
Certifications, Licenses, and Registrations
Required:
Current, unencumbered Registered Nurse license in the state of employment.
Required:
National Provider BLS – American Heart Association.
Physical Demands And Work Environment The role requires standing, walking, manual dexterity, and the ability to lift/move objects up to 50 pounds. Regular vision requirements apply. Exposure to typical healthcare environment conditions including bodily fluids and infectious diseases. Ability to use standard clinical equipment. Reasonable accommodations can be made for individuals with disabilities.
Compensation Compensation range:
$45.73-$71.00 per hour. The listed base pay may be supplemented by shift differentials, on-call, overtime, bonuses, or other incentives.
