Home Health Registered Nurse (RN)
to provide expert care in patients’ homes, where healing happens best. You’ll work both independently and as part of an interdisciplinary team to conduct comprehensive assessments, develop and implement individualized care plans, and deliver outcome-driven interventions under the physician-ordered Plan of Care. This role is rooted in the principles of community health nursing and emphasizes serving patients and families with skill, heart, and professionalism. The RN is accountable for the delivery and supervision of safe, quality care that is individualized for assigned patients and follows the nursing process in accordance with the Nurse Practice Act (assessment, planning, implementation, evaluation). The role supports a collaborative, patient- and family-centered approach within a dynamic healthcare environment. Position Specific Responsibilities
Possesses knowledge of developmental needs and the assessment and treatment as it relates to the age of the patient population served. Conducts and documents a comprehensive patient assessment upon admission. Continually reassesses and applies appropriate advocacy and interventions as indicated. Interprets a patient’s self-assessment and behavior with understanding of cognitive, physical, emotional, psychosocial, and maturation factors. Collaborates with the patient, family and healthcare team; develops, documents, implements and monitors an individualized plan of care reflecting best practice and evidence-based medicine. Adapts to change and updated patient care practices within an evolving healthcare environment. Documents patient care activities accurately and timely, both on paper and electronically where applicable. Supports and assists with orientation of new staff and students. Acquires necessary training to competently perform preceptor and charge nurse functions when selected for those roles. Demonstrates ongoing professional nursing practice through continuing education, required training, certification, and department participation. Fosters open communication and patient- and family-centered care while maintaining healthy collegial relationships. Demonstrates timely communication based on individual patient/family needs. Attends or reviews minutes for a minimum of 50% of scheduled staff meetings and stays accountable for meeting content. Education and/or Experience
Required - Associate or Bachelor's degree from an accredited School of Nursing. Preferred - 1 year of recent experience in home health, acute care, or a skilled nursing facility. Certifications, Licenses, and Registrations
Required - Current, unencumbered RN license in the state of employment. Required - National Provider BLS - American Heart Association. Physical Demands and Work Environment
The demands of this role may require adapting to accommodate team members with disabilities. Regular standing, walking, and manual dexterity; ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. Exposure to bodily fluids, infectious diseases, and typical healthcare conditions is expected. Routine use of standard medical equipment and tools is essential. Compensation range:
$45.73-$71.00 per hour Notes:
The base pay range may be supplemented by shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities. Benefits:
