This Pharmacist (Clinical Pharmacy Specialist) is in Geriatrics and Extended Care services within the White River Junction VA Health Care System located at White River Junction, Vermont. Duties

The Clinical Pharmacy Specialist (CPS) functions at the highest level of clinical practice, works independently under their individual scope of practice to directly care for patients. A CPS functions as a member of their assigned team and serves as an advanced practice provider to initiate, modify, or discontinue medication therapy and as a consultant for intensive medication therapy management services. Designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient-centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Responsibilities

Customer Service: Treats patient, team members, other facility employees and visitors with respect. Responds to the needs of the Veteran and staff in a courteous and timely manner. Demonstrates interpersonal skills to achieve the goals of the service. Acts as a team member and is supportive of the group's efforts. Clinical - Direct Patient Care: Core functions include patient care encounters (face-to-face and virtual) with referred patients for comprehensive medication management in practice areas identified in their scope of practice. Design, implement, monitor therapeutic drug plans; order, perform, review, and analyze laboratory tests to monitor therapy; follow-up with patients on lab results; determine if referral to higher care is necessary when interventions fall outside CPS scope. Clinical - Indirect Patient Care: Identify patients who may benefit from comprehensive medication management; review regimens for effectiveness, contraindications, interactions, and outcomes; communicate findings to prescribers; monitor for drug errors and adverse reactions; promote compliance with drug therapy policies; support compliance with initiatives. Education and Research: Participate in training of pharmacists, technicians, students, and residents; serve as a preceptor per policy. Drug Information: Provide timely, accurate drug information to staff and students; instruct on medication use and devices. Performs additional duties as required. Work Schedule

Applicants pending completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship; Education (doctor of pharmacy or equivalent), licensure, and English language proficiency. Foreign education considerations apply with appropriate certification. Grade Determinations

GS-13 Clinical Pharmacy Specialist: One year of experience at GS-12 level or equivalent, plus demonstrated KSAs including ability to communicate persuasively, understanding of regulatory/quality standards, problem-solving and coordination, expert knowledge in a clinical pharmacy area, and advanced skill in monitoring drug therapies. Also includes KSAs for GS-12 level (professional practice, communication, laws/regulations, and monitoring outcomes). Preferred Experience

Experience in a home-based care setting. Experience with Comprehensive Medication Management as a Clinical Pharmacist Practitioner (CPP). Residency in geriatrics or experience managing geriatric patients. Demonstrated ability to manage patients with complex care needs, particularly in rural areas. Education and Additional Information
