Job Overview Clinical Pharmacy Specialist (CPS) to provide services at our Community Based Outpatient Clinic. This role focuses on drug monitoring and management of therapy with emphasis on diabetes, hypertension, and dyslipidemia. The CPS is an integral part of the team assigned to a panel of patients to support continuity of care and to coordinate and provide clinical pharmacy services to patients and staff at the clinic.

Essential Functions

The CPS functions at the highest level of clinical practice, works independently under their scope of practice to directly care for patients.

Plays a defined role in budgetary execution and serves as a mid-level provider who initiates, modifies or discontinue medication therapy and as a consultant for intensive medication therapy management services.

Design, implement, assess, monitor and document therapeutic plans utilizing the most effective, least toxic and most economical medication treatments.

Help achieve positive patient-centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas.

Perform physical assessments.

Order laboratory and other tests to help determine efficacy and toxicity of medication therapy.

Required Skills

Minimum of one (1) year of full-time experience within the past three (3) years as a provider in a related area of clinical pharmacology.

Previous experience managing pharmaceutical care to patients in the outpatient setting is preferred.

Current CA state unrestricted license as a Clinical Pharmacist. Must comply with all state requirements and regulations.

Maintain current CPR/BLS from American Heart Association or Military Training Network.

Ability to communicate orally and in writing to persuade and influence clinical and management decisions.

Expert understanding of regulatory and quality standards for their program area.

Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.

Expert knowledge of a specialized area of clinic pharmacy practice or specialty area of pharmacy.

Advanced skill in monitoring and assessing the outcome of drug therapies, including interpretation of laboratory and other diagnostic parameters.

Required Education and Experience

Graduate of an accredited Doctorate program in pharmacology (Pharm.D), preferably with an ambulatory care residency, or equivalent experience demonstrating competency. ACPE-accredited Pharm.D. or BS Pharmacy with post-graduate experience in chronic disease state and medication management services.

Must meet licensed qualification standards to include graduation from an approved college or university and post-graduate experience performing chronic disease state and medication management services (e.g., post-graduate residency or direct patient care under collaborative practice agreement or similar experience).

Working Conditions / Physical Requirements

Moderate physical activity. Ability to lift up to 25 pounds and/or stand/walk for extended periods.

Typically interior/office work environment.

Respond to medical emergencies within the clinic as needed.

Pay rate: Starting at $65/hour
