Job Summary Care Partners are responsible for meeting the basic needs of patients by providing skilled levels of bedside patient care under the direction of the Registered Nurse. Care Partners also perform clerical duties necessary to the functioning of the nursing unit.

Responsibilities and Requirements POSITION ACCOUNTABILITIES AND PERFORMANCE CRITERIA (% of Time) — Essential Functions: The following are essential job accountabilities and performance criteria.

General Patient Care: Carries out the patient’s plan of care as it relates to physical needs under the direction of the Registered Nurse; delivers care requiring broad knowledge of aseptic technique, medical terminology and equipment; assists nurses and physicians with procedures; maintains knowledge of and operates equipment used in patient care safely; retrieves patient care items not routinely available on the unit; retrieves, recovers or follows up on patient care needs; completes unit clerical duties including coordination of designated unit activities, transcription of orders, medical record maintenance and serving as unit receptionist; prepares rooms prior to admissions and following discharges; completes patient’s belongings list on admission and transfer; directs prompt and courteous attention to patients, families, and visitors; rounds with purpose; engages in service recovery activities; rounds after dietary trays; takes action to resolve issues related to late or wrong trays; feeds and sets up trays as needed; maintains supplies and equipment; notifies appropriate departments to maintain a clean, safe environment; functions as a resource person to co-workers, patients/families, medical staff, and ancillary personnel.

Safety and Comfort: Correctly places restraints; observes for signs of constriction, skin changes, breathing; provides ROM and skin care, toileting, and opportunities for socialization; operates cardiac monitor components as directed; monitors vital signs and related measurements; obtains strip and related data; completes point-of-care testing as approved; weighs and measures patients; tracks intake and output; mobility and exercise support including applying compression stockings and ambulation.

Respiratory and Wound Care: Assists with incentive spirometry and cough/deep breath; sets up oxygen therapy as directed; monitors pulse oximetry; performs tracheostomy care and suctioning; wound care including dressing changes and monitoring pin sites.

Bowel, Bladder, Hygiene and Specimens: Administers enemas; performs straight catheterization and catheter care; provides hygiene care and daily grooming; performs mouth care; ensures regular hydration; collects specimens (urine, sputum, blood, stool) as required; processes specimen labeling and chart documentation.

Clerical and Communication: Answers phones promptly; provides directions to physicians, hospital personnel and visitors; notifies staff of patient arrivals; assists in receiving items; handles pneumatic tube; maintains daily worksheets and census; updates patient and transfer/discharge documents; transcribes orders and communicates with laboratory, radiology, pharmacy and other departments as needed; ensures correct labeling of forms; provides shift reports and hand-off communication; ensures complete and accurate charting; supports nursing staff with paperwork for transfers and discharges.

Service and Professionalism: Delivers service excellence with timely, high-quality care; demonstrates care and courtesy; responds to call lights promptly; maintains patient confidentiality; communicates effectively and collaborates with the care team; uses resources wisely; participates in performance improvement and quality initiatives; demonstrates personal accountability and participates in unit goals.

Qualifications and Requirements MINIMUM EDUCATION:

High School graduate or GED; at least 18 years of age. Post-high school vocational/specialized training preferred.

MINIMUM EXPERIENCE:

None

REQUIRED COURSE:

Completion of learning program with 80% passing score on written and skills performance examination.

Work Environment

Regular exposure to blood-borne diseases; contact with patients in varied circumstances; exposure to accidents, injuries, and illness; subject to irregular hours and high-pressure demands.

Physical Requirements

Prolonged standing/walking; ability to lift, position, push, and transfer patients; lift supplies/equipment; considerable reaching, bending, kneeling, and lifting 100+ pounds.

Licenses/Certifications Required:

Basic Life Support (BLS HCP) from American Heart Association; state requirements may vary by location. Other resuscitation certifications may be required dependent on department.
