Regular Job Summary:

Care Partners are responsible for meeting the basic needs of patients by providing skilled levels of bedside patient care under the direction of the Registered Nurse. Care Partners also perform clerical duties necessary to the functioning of the nursing unit. Job Responsibilities

General Patient Care Carries out the patient’s plan of care as it relates to the patient’s physical needs under the direction of the Registered Nurse. Delivers care requiring broad knowledge of aseptic technique, medical terminology and equipment. Assists nurses and physicians with procedures. Maintains knowledge of and operates equipment used in patient care safely. Retrieves patient care items not routinely available on the unit. Retrieves, recovers or follows up on patient care needs. Performs unit clerical duties including coordination of designated unit activities, transcription of orders, medical record maintenance and serving as a unit receptionist. Prepares rooms prior to admissions and following discharges. Completes patient belongings list on admission and transfer. Directs prompt and courteous attention to the needs of patients, families, and visitors. Rounds with purpose and engages in service recovery activities; refer unusual problems to Nurse Manager/Designate. Rounds after dietary trays to ensure meals are appropriate or as required for patient satisfaction/care needs; take action as necessary for late or wrong trays. Feeds and sets up trays as needed; maintains supplies and equipment. Notifies appropriate department to maintain a clean safe environment (housekeeping, maintenance, dietary, etc.). Functions as a resource person to co-workers, patients/families, medical staff, and ancillary personnel. Safety and Comfort Correctly places restraints; observes for constriction of circulation, changes to skin integrity, adequate breathing; positions appropriately; provides ROM and skin care, toileting, and socialization opportunities. Operates cardiac monitor as directed (on/off, alarms/stand-by, obtaining strip, lead placement). Vital Signs/Measurement Completes Point of Care Testing as approved by hospital and competency. Obtains vital signs including temperature, measures abdominal girth, weighs patients, and measures intake and output. Mobility and Exercise Applies Pneumatic Air Stockings / Ted Hose. Ambulates, turns and positions patients. Respiratory Care Assists with incentive spirometry and cough/deep breath. Sets up oxygen therapy (nasal cannula or mask) as directed by Registered Nurse. Monitors pulse oximetry; completes tracheostomy care and suctioning as capable. Wound Care Places patient on specialty bed; performs dressing changes (sterile and clean). Cares for pin care site after RN assessment; monitors traction weights. Bowel and Bladder Elimination Administers enemas; performs straight catheterization and catheter care. Hygiene Provides hygiene and grooming; mouth care; fresh water at scheduled times; perineal care; toileting. Specimen Collection Obtains various specimens as listed (urine, sputum, blood, stool) per protocol and unit requirements. Clerical Answers phone promptly, directs visitors, notifies physician of new admissions, assists with receiving items, handles pneumatic tube, maintains daily worksheet, verifies census, communicates unit activities, ensures proper specimen labeling, reports critical labs, completes pre-op and admission data, assists in transfers/discharges, provides hand-off reports, transcribes orders, enters orders, and maintains charting accuracy. Safety/Infection Control Follows safety and infection control policies; maintains safe, clean environment; completes annual mandatory requirements; communicates changes in patient condition; documents concisely; identifies problems and participates in PI and unit goals; handles linen and safety responsibilities. Service Delivers timely, high-quality care in a courteous, respectful manner; demonstrates care and courtesy, effective communication, confidentiality, competence, collaboration, and cost-effective service; engages in continuous improvement. Personal Accountability & PI Demonstrates accountability, participates in learning experiences, and contributes to unit goals; participates in performance improvement processes; collects PI data as assigned; shares knowledge and supports quality initiatives. Age Appropriate Care Provides age-appropriate care across patient populations typical to the department; demonstrates knowledge of growth and development and involves family in decision making. Specialized Care Provides specialized care to high-risk patients including restraint care, pain management, abuse assessment, and related criteria. Job Specifications MINIMUM EDUCATION: High School graduate or GED equivalent and at least 18 years of age. Post High School vocational/specialized training preferred. MINIMUM EXPERIENCE: None REQUIRED COURSE: Completion of learning program with 80% passing score on written and skills performance examination. WORKING CONDITIONS: Regular exposure to the risk of blood-borne diseases. Contact with patients under wide variety of circumstances. Exposure to unpleasant elements (accidents, injuries, illness). Subject to trying and unpredictable situations. May perform emergency care. Subject to irregular hours. Pressure due to multiple competing demands. PHYSICAL REQUIREMENTS: Prolonged standing/walking; lifts/transfers patients and lifts supplies/equipment; considerable reaching, stooping, bending, kneeling, crouching; lifting up to 100+ pounds.
