Overview

Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients in multiple states. Ennoble Care’s clinicians visit patients in the home, providing continuum of care for those with chronic conditions and limited mobility. The organization offers programs including remote patient monitoring, behavioral health management, and chronic care management. The team is driven to make a difference and operates under the motto, “To Care is an Honor.” The Hospice Clinical Director is responsible for ensuring the coordination and management of patient care, assisting the Director of Nursing in maintaining the quality of patient care in accordance with the Agency’s objectives and policies through planning, coordination, implementation and evaluation of hospice services. This market services the Tulsa region in Oklahoma. Responsibilities

Coordinate and oversee direct and indirect patient services provided by clinical personnel. Provide supervision and guidance to clinical staff to ensure quality of services and maintenance of standards. Perform chart audits. Gather data for QAPI and participate in QAPI activities. Appraise clinical staff performance during probationary periods and annually; provide teaching and counseling to enhance service delivery and professional development of nursing and health aide staff. Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assigning direct care staff; assist with orientation of new staff; see patients in the home setting as needed; maintain adequate and appropriate medical supplies. Assist with orientation of new personnel and in-services for current staff. Ensure Medicare, State and ACHC regulations are followed by all clinical staff. Assist the Director of Nursing with responsibilities as requested. Commit to self-development and continuous improvement of nursing/management practice through formal education, workshops, conferences, professional meetings, and/or individual research. Job Conditions

Position may be stressful. Primarily a desk job with occasional lifting of medical supplies and charts. May require bending, stooping, reaching, and moving patient weight up to 250 pounds; lift up to 30 pounds. Travel required to patient homes when supervision of care is scheduled. Access to patients’ homes may not always be wheelchair accessible. Hearing, eyesight and physical dexterity sufficient to perform physical assessments and patient care. Use of computer, multi-line telephone, calculator, copier, and other office and medical equipment. Company Information

Has access to patient medical records, patient financial accounts, and department staff records. Qualifications

Must be a graduate of an accredited School of Nursing. Must be a Registered Nurse licensed in the state of OK. The Clinical Supervisor must possess at least one of the following: A Bachelor of Science in Nursing with two (2) years combined public health nursing and progressive professional responsibilities in public health nursing OR three (3) years combined public nursing and progressive professional responsibilities in public health nursing. Minimum of 2 years nursing experience in home hospice or skilled home care (hospice preferred). Ability to market and interact tactfully with customers and the community. Knowledge of business management, governmental regulations and ACHC standards. Strong verbal and written communication skills; ability to demonstrate self-confidence and maintain enthusiasm toward goals. Benefits (Full-time employees)
