Overview
Hospice Clinical Director serving Morris / Sussex County, NJ. The market services the Hackettstown region in New Jersey and surrounding areas. Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients across multiple states. Clinicians visit patients in their homes to provide continuum of care for those with chronic conditions and limited mobility. Ennoble Care offers programs including remote patient monitoring, behavioral health management, and chronic care management. Coordinate and oversee direct and indirect patient services provided by clinical personnel. Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards. Gather data for QAPI and participate in QAPI activities. Appraise clinical staff performance during probationary and annual periods; provide teaching and counseling to enhance delivery of services and the professional development of nursing and health aide staff. Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff. Assist with the orientation of new staff. See patients in the home setting as needed. Maintain adequate and appropriate medical supplies. Assist with orientation of new personnel as well as in-services for current staff. Ensure Medicare, State and ACHC regulations are followed by all clinical staff. Assist the Director of Nursing with responsibilities as requested. Pursue self-development by continuing education, attendance at workshops and conferences, active participation in professional associations, and/or independent study. Job Conditions
Position may be stressful. Primarily a desk job with minimal and occasional lifting of medical supplies and charts. Occasionally bend, stoop, reach, and move patient weight up to 250 pounds and lift/carry up to 30 pounds. Travel to and from patients’ homes when supervision of care is scheduled. Ability to access patients’ homes which may not be wheelchair accessible. Hearing, eyesight, and physical dexterity sufficient to perform physical assessments and patient care. Use of computer, multi-line telephone, calculator, copier, and other office/medical equipment. Company Information

Graduate of an accredited School of Nursing. Registered Nurse licensed in the state of NJ. Clinical Supervisor must possess at least one of the following: a Bachelor of Science in Nursing with two (2) years combined public health nursing and progressive professional responsibilities in public health nursing, or three (3) years combined public nursing and progressive professional responsibilities in public health nursing. Minimum of 2 years nursing experience in home hospice or skilled home care (hospice experience preferred). Ability to market and interact tactfully with customers and the community. Knowledge of business management, governmental regulations, and ACHC standards. Strong verbal and written communication skills; self-confidence and a positive attitude toward achieving goals.
