Overview
Job Title: Primary Clinician/Outreach and Recruitment Coordinator. The Research Foundation for Mental Hygiene, Inc. (RFMH) is seeking to fill the full-time position on the OnTrackNY team at the South Beach Psychiatric Center Coney Island/Sheepshead Bay clinic. The program serves adolescents and young adults within two years of onset of non-affective psychosis. The position is 1.0 FTE as part of a small multidisciplinary team providing coordinated and assertive treatment in the earliest stages of illness. Responsibilities
Recruit patients for the program through community education and presentations at potential referral sources. Serve as the primary contact for referral sources. Establish and maintain a database of referral sources with up-to-date contact information. Conduct telephone and in-person eligibility screening of referred patients. Refer patients ineligible for the program to other appropriate community providers. Attend weekly team meetings and participate in ongoing training and supervision. Rotate on-call responsibilities with other team members. Serve as the Primary Clinician for a subset of program participants and as the point person for participants and families. Provide services in the community (off-site) as needed based on participant and family preferences. Conduct needs assessments, safety planning, and wellness management planning with participants. Connect participants and families with needed services and supports; provide behavioral interventions and actively support the family domain. Conduct clinical assessments to determine social functioning and substance use using a shared decision-making framework. Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psychoeducation. Provide psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program; offer supportive therapy. Develop and modify treatment using a shared decision-making process with clients. Support transition planning from the team to other services and ensure stable links at discharge. Minimum Qualifications
Licensed Master’s or Ph.D. in an appropriate field (social work, psychology, mental health counseling). At least 1 year of experience as a clinician providing recovery-oriented, community-based mental health services. Preferred Qualifications
Two or more years of experience working with adolescents and young adults experiencing psychosis. Work Location
