Overview

LVN - Outpatient - Level II – Meridian Medical Office Build – Family Medicine-Regular Clinic – On Call (Bilingual Spanish) Provides nursing care to patients under the direction of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills to observe nursing intervention and assists medical staff to ensure optimal care within established policies, procedures, and standards. Under the direct supervision of a Registered Nurse or provider, provides nursing care in accordance with the legal scope of practice and within established standards of care, policies, and procedures. Demonstrates the Mission, Vision, and Strategic Plan of the organization and treats coworkers, patients, families, and all health care team members with dignity and respect. Responsibilities

Exhibits excellent customer relations with patients, visitors, providers, and co-workers; shows courtesy, compassion, and respect; conforms to the critical service standards established for the department. Provides therapeutic patient care in accordance with the patients’ plan of care established by physician/advanced practice providers and/or a registered nurse within the scope of practice. Transcribes physician/advanced practice providers orders and directed by policy. Communicates directly with a registered nurse to institute changes in the plan of care; assists with and contributes to the development of a nursing care plan. Administers specified medications appropriately and accurately following the provider’s order and/or standing orders within scope of practice. Executes treatments such as urinary catheterizations, placement of intravenous catheters, bladder irrigations and instillations, suctioning procedures, throat irrigations and enemas, and utilizes equipment as required; adheres to infection prevention and control policies and procedures. Adheres to high standards of disinfection and usage policies for all clinical areas and equipment. Performs related patient care functions such as taking vital signs, wound care, patient education, obtaining specimens, and ensuring correct culture collection. Assists physician/advanced practice providers or other medical staff with examinations and treatments by selecting and passing instruments, providing support and restraint to the patient, draping, and other assistance as directed or required. Observes and documents patient condition, initiates appropriate emergency procedures as necessary, and reports unusual conditions to the RN/APN/physician as established by policy and procedure in a timely manner. May volunteer to start and maintain IV therapy and venipuncture with certifications; notifies the RN/APN/physician of any problems. Collects and processes specimens per policy, procedures, guidelines, and protocols. Provides non-composited teaching following the RN/APN/physician plan for education. Reports identified learning needs and discharge barriers. Ensures cleanliness of nursing module/station, patient exam room, and proper functioning of patient care equipment; provides a safe and therapeutic environment for patients and staff. Utilizes effective verbal and non-verbal communication. Performs in other departments or modules as assigned to maintain established standards of care and safety. Performs departmental back-office workflows including proactive office encounters, closing care gaps, patient outreach, access and/or nurse visits. Participates in staff meetings and in-service programs. Assists in ordering and distribution/stocking of supplies and equipment for the module/department. Accepts verbal and telephone orders from the physician/advanced practice providers. Supports patient communication and message management including patient emails, call center messages, and other back-office messages. Documents care in Health Connect per policy, procedures, guidelines, and protocols. Assists in coordinating the discharge process and performs all job functions within license certification and scope of practice. Promotes, ensures, and improves customer service to internal/external customers by demonstrating skills consistent with the organization’s philosophy of providing extraordinary customer relations and quality service. Qualifications

Licensed Vocational Nurse (LVN) in California with current licensure and appropriate certifications as required by the position. Ability to work under the supervision of a Registered Nurse or physician/advanced practice provider within the scope of practice. Experience in outpatient/clinic settings and proficiency in clinical procedures listed in responsibilities.
