LVN -Outpatient- Level II - Meridian Medical Office Build - Family Medicine-Regular Clinic - On Call (Bilingual Spanish)
Job Summary Provides nursing care to patients under the direction of a Registered Nurse or physician/advanced practice provider. Utilizes observational skills to assess nursing intervention and assists medical staff to ensure optimal care within established policies, procedures, and standards. Under the direct supervision of a Registered Nurse or provider, provides nursing care in accordance with the legal scope of practice and within established standards of care, policies, and procedures. Demonstrates alignment with the Mission, Vision, and Strategic Plan of the organization and treats coworkers, patients, families, and all members of the health care team with dignity and respect. Exhibits excellent customer relations with patients, visitors, providers, and co-workers; demonstrates courtesy, compassion, and respect; adheres to the department’s critical service standards. Provides therapeutic patient care in accordance with the patient’s plan established by physicians/advanced practice providers and/or a registered nurse within the scope of practice. Transcribes physician/advanced practice providers orders and follows policy directions. Communicates directly with a registered nurse to adjust the plan of care; contributes to the development of a nursing care plan. Administers specified medications appropriately and accurately following the provider’s orders and/or standing orders within the scope of practice. Performs treatments such as catheterizations, IV catheter placement, bladder irrigations and instillations, suctioning, throat irrigations and enemas, and uses required equipment; adheres to infection prevention and control policies. Maintains infection control and disinfectant policies and procedures; ensures proper use of PPE and procedural hygiene. Performs related patient care tasks (e.g., taking vital signs, wound care, patient teaching, specimen collection) and assists with procedures as directed. Assists physicians/advanced practice providers or other medical staff with examinations and treatments by preparing instruments, supporting the patient, draping, and other required assistance. Monitors patient condition and documents signs, symptoms, and reactions to therapy; identifies unexpected findings and reports to the RN/APN as required by policy. May assist with IV therapy and venipuncture with appropriate certifications; may discontinue IV therapy as directed. Collects and processes specimens per policy and protocol. Provides non-complicated teaching following the care plan; reports learning needs and discharge barriers. Maintains cleanliness of nursing station and patient areas; ensures safe and therapeutic environment for patients and staff. Communicates effectively using verbal and non-verbal methods. Performs duties in other departments or modules as assigned to maintain standards of care and safety. Supports back-office workflows including care gap closure, patient outreach, and nurse visits. Participates in staff meetings and in-service programs; assists with supplies and equipment management. Accepts verbal and telephone orders from physicians/advanced practice providers. Supports patient communication and message management (emails, call center messages, back-office messages). Administers pulse oximetry, humidification therapy, spirometry, oxygen therapy, suctioning, and related procedures as ordered. Documents care in Health Connect per policy and procedure. Performs other related duties as assigned, including orientation of new employees and coordinating discharge processes. Performs all job functions within license certification and scope of practice; promotes customer service aligned with the organization’s standards. Contributes to compliance with the Principles of Responsibilities and privacy/confidentiality requirements. Age-related competencies: ensures care for different age groups according to their needs. Experience Minimum twelve (12) months LVN direct patient care experience within the last three (3) years. Experience may include medical office, acute care, home care services, or skilled nursing facility. Education High School Diploma or GED License, Certification, Registration Vocational Nurse License (California) Basic Life Support Additional Requirements Extraordinary customer relations and quality service are essential. Bilingual (English/Spanish) Level II required. Preferred Qualifications IV Certification and Blood Withdrawal Certification preferred. Notes Schedule includes all shifts, rotating weekends, holidays, and floating within the established seniority area. Must successfully pass or be active in the bilingual test (within the last 12 months) or be active in the QBS program.
