is a Colorado 501(c)3 nonprofit organization located in Colorado Springs, founded in 1998 to provide respite and specialized care for children and adults with disabilities and special needs. Various programs offer community support services to families designed to meet the needs of individuals with disabilities throughout their life span. Scope

The Host Home Program Director oversees the Host Home/Residential Services Department, including recruitment of host home providers and clients, ensuring that the rights of the clients are being met, implementing the regulatory and safety standards of the agency, the Division for Intellectual and Developmental Disabilities and the Community Center Boards. This position directly supervises the Residential Services staff and reports to the Executive Director. Ensure that all applicable standards as per Division of Intellectual and Developmental Disabilities (DIDD) Rules and Regulations are implemented and audited regularly so that clients receive quality services. Receive & Review all residential services provider applications and clients’ referrals. Follow the agency policies and procedures to include Residential Program Nurses and obtain all necessary admission documents. Program acceptance of future providers and client referrals will have the final approval of the Executive Director. Coordinates the Host Home provider training program implementation per Division of Intellectual and Developmental Disabilities (DIDD) Rules and Regulations. Supervise Host Home Supervisors and Residential Nurses, including ongoing auditing to ensure timelines for all assigned tasks are completed promptly and professionally and schedule Residential Services team meetings. Meet regularly with the Executive Director to address issues around program management, needed updates on internal audit findings, operations/procedures, and supervision of Residential Services Supervisors, regulations, and program marketing. Effectively communicating critical incidents, concerns, and needed policies/procedures to the Executive Director. Develop and maintain professional relationships in the community, and increase collaboration with other agencies and/or businesses to enhance the overall program growth, including attending monthly provider meetings. Conduct random monitoring visits according to the Host Home Supervisors Monitoring Schedule to ensure monitoring’s are being completed and that there is a written follow-up plan to address any deficiencies identified and how the deficiencies will be corrected. Ensure corrections are made on time. Coordinate and oversee the medical, dental, behavioral, psychiatric, and other needed services for the clients with the Residential Nurse and Host Home Supervisors. Work with the Host Home Department staff in overseeing and assuring that all documentation regarding Health and Safety Assessment, Quarterly and Annual Reports and Assessments, ISSPs, BISSP’s Comprehensive Life Reviews, Functional Analysis when needed, case notes, and any other documentation necessary for HRC and/or IDT team are fully completed and submitted to the appropriate individuals in a timely manner. Will attend HRC review and HRC Liaison meetings. Oversee the handling of all incidents following the Agency’s reporting policies and procedures and at all times taking into consideration of the health and safety of the clients, reporting critical incidents timely to the Executive Director. Ensure that each client’s Agency File is completed and up to date at all times. This includes the Client’s Home Book which is located at the Host Home residence. In conjunction with the Host Home Program Coordinator, problem-solve any issues/concerns related to the IDT and/or Host Home Provider and ensure that recommended actions are consistent with agency policies, procedures, and protocols. Develop and maintain an emergency on-call response system for the clients in the host home settings, working with the Host Home Supervisors, and implementing the on-call rotation. Monthly, oversee the accuracy of Personal Needs Funds cash ledgers and inventory, and submit all necessary paperwork to the Finance Manager. Promote the Agency in the community and with agency events. Work with the Executive Director in the development and implementation of any agency Policies and Procedures changes, including any IDD and/or TRE regulatory changes. Other related duties as assigned. Minimum Qualifications

Requires a Bachelor’s degree, preferably in a Human Services field. Requires a minimum of two years of experience in serving individuals with developmental disabilities as a Residential Services Supervisor. Prefer 1–2-year experience as a Host Home Coordinator or department team leader. Strong leadership abilities and problem-solving skills are required. CPR, First Aid & CPI, & Universal Precautions Certification within the first 30 days of hire. A valid driver’s license & current vehicle insurance, and an acceptable driving record per SKSF standards. Salary

SALARY:

$60,000-$65,000/yr. DOE; full-time/exempt position Benefits Include
