Overview The Program Director directs, manages, and administers the coordination and service delivery components of programs. Monitors and/or participates in the implementation and delivery of the Individual Service Plan (ISP). Supervises the delivery of services and ensures strategies reflect the goals and objectives of the ISP. Responsible for supervising the formulation of the participant’s plan for the delivery of all services provided. Monitors services that address health and safety risks for the participants. Responsibilities Builds, develops, and effectively manages the operations of QIDP/Program Coordinators to optimize individual and team performance through effective leadership, mentoring, and training. Assists the Executive Director and/or Associate Executive Director with timely, open, and effective communication regarding persons served. Ensures preplacement processes and paperwork are reviewed and implemented. Ensures annual staffing is completed within 365 days, with assessments completed within 30 days of the staffing, including comprehensive functional assessments. Implements new training objectives based on IDT discussions and assessments. Ensures IPCs are renewed within 60 days of expiration and IPs are completed upon receipt of PDP from service coordination. Updates comprehensive functional assessments based on changes with an individual, with IDT documentation noting changes. Ensures Plan of Corrections are properly completed, corrected, and submitted within the identified timeframe. Monitors monthly training objectives to ensure active treatment is continuous. Completes quarterly reports as required. Provides Q/PC training to DSP staff on the use of AA, diets, training objectives, and BSPs. Maintains staffing calendars, updating as admissions and discharges occur, tracking annual assessments, and documenting completion. Ensures BSPs are written, behavior data sheets are in place, and monitored by QIDP/Program Coordinators. Reviews IBAMs and triage reports daily, with follow-up in daily huddles. Runs weekly reports in TMP to identify trends within IBAMs. Conducts weekly IDT meetings with QIDP/PC/LVN to review medical consults, ER visits, and hospital discharges. Submits monthly incident information to OSS. Reviews medical appointments, ER visits, and hospitalizations with LVN/RN weekly, and holds IDT meetings as needed. Ensures ICF-HRC meetings are held at least twice a year to review rights restrictions, obtaining HRC approval as needed. Ensures HCS-CAC meetings are held quarterly to review required components, with CAC approval as needed. Ensures appropriate informed consent is obtained for psych meds and rights restrictions, if needed. Performs chart audits, follows up on chart audits, pre-surveys, and plans of correction. Maintains current information regarding individuals in the home and in TMP. Schedules Qs/PCs to monitor homes, including active treatment, medication, and meal observations, with follow-up as needed. Ensures ICAPs are completed and submitted to CBC 60 days before expiration. Reviews LONs and staffing patterns annually to ensure alignment. Implements service strategies, including Q/PC training, TMP training, and BSP training (QIDP only). Uploads all programming paperwork into TMP. Conducts monthly site visits and client chart reviews for accuracy and compliance. Completes and submits all financial documents timely. Enters all critical incidents into the incident management system. Analyzes consumer care information and develops recommendations to address concerns by meeting regularly with supervisors, professionals, and managers. Performs regular quality assurance reviews of charts, documentation, and MARs to ensure service delivery meets individual needs and goals. Observes service delivery on-site, monitoring knowledge of ISP requirements, health needs, behavior management, and emergency procedures. Serves as a liaison between the community, agencies, family/guardians, and the agency. Informs AED/ED of critical incidents and clinical concerns. Coordinates investigations of serious incidents and abuse allegations, including reporting to agencies. Serves as on-call support for person-supported issues. Monitors compliance with financial goals to ensure services align with each person’s IPP. Coordinates incident/accident reviews, monitors patterns, and provides feedback to prevent future incidents. Serves on or chairs Human Rights, Consumer Advisory, and Safety Committees and manages admissions with the placement committee. Develops and implements plans of correction. Participates as a member of the agency management team. Ensures QIDPs/PCs are hired, trained, evaluated, and monitored. Completes performance reviews timely to ensure employee productivity and accountability. Ensures staff operate within fair labor practices, policies, and regulations. Collaborates with the team to ensure timely completion of plans of correction. Conducts regular staff meetings to share information and develop action plans. Provides and monitors staff training in least restrictive techniques, behavior management, active treatment, client rights, abuse prevention, documentation, emergency procedures, and other relevant areas. Qualifications Bachelor’s Degree in human services or a related field. Minimum of two years of direct experience with persons with developmental disabilities. Minimum of two years of supervisory experience. Ability to meet DDP qualifications, demonstrate excellent communication skills, and work flexible hours. Valid driver’s license with a satisfactory driving record as per company policy. Minimal travel required.
