Job Title: Primary Care Physician – PACE Program Location: El Paso, TX Job Type: Full-Time Schedule: Monday – Friday, 40 hours per week Salary: $250,000 annually Department: Medical Services Reports To: Medical Director About the Organization: Join a mission-driven Program of All-Inclusive Care for the Elderly (PACE) organization dedicated to delivering high-quality, comprehensive care to frail and elderly individuals who wish to live independently in their communities. Position Summary: We are seeking a compassionate, board-certified Primary Care Physician to provide high-quality, comprehensive, and coordinated care to our PACE participants in El Paso, TX. This physician will play a critical role in the interdisciplinary care team, delivering care across clinical settings including the PACE center, participant homes, and contracted facilities. Responsibilities
Provide direct primary and preventive care to PACE participants. Perform comprehensive assessments, diagnosis, treatment, and management of acute and chronic conditions. Participate in and collaborate with the Interdisciplinary Team (IDT) to develop and update individualized care plans. Oversee and coordinate all aspects of medical care including specialty services, hospital admissions, and transitions of care. Conduct home visits and provide care in the day center and nursing facilities as needed. Participate in on-call coverage on a rotational basis. Educate participants and families on health conditions, medications, and treatment plans. Ensure compliance with federal and state regulations as well as PACE program guidelines. Qualifications
Medical Degree (MD or DO) from an accredited institution. Board Certification in Internal Medicine, Family Medicine, or Geriatrics. Current, unrestricted Texas medical license. DEA registration and current CPR certification. Minimum of 1 year of experience working with frail or elderly populations preferred (PACE experience highly desirable). Strong interpersonal, communication, and organizational skills. Ability to work collaboratively in a team-based, person-centered care environment. Compensation & Benefits
