Overview

This is an exciting opportunity to be part of a dynamic dental team that is providing oral health services. The dental program is an integral component of a fully integrated health home that includes primary care, mental health, and public health services. The Community Health Center of Benton and Linn Counties (CHC) is a growing Federally Qualified Health Center (FQHC) with clinic sites in Corvallis, Monroe, Lebanon, and Sweet Home Oregon. Regular clinic hours are 8 AM to 5 PM, Monday – Friday. The Health Services Building in Corvallis includes a three operatory dental suite for adult patients. The Boys & Girls Club Dental Clinic, also known as the Johnson Dental Clinic, serves children and expecting mothers. We offer an extensive outreach, prevention and education program that includes screening, fluoride varnish, sealants and silver diamine fluoride at all Benton County and East Linn County schools, WIC, and Head Starts, as well as adult prevention services provided at primary care clinics, community settings, and assisted living facilities. This position provides high quality, direct patient care according to training, experience, and licensure. Dental services are provided in a variety of settings, including dental clinics, medical clinics, schools, and community settings. Our dentists are part of the dental team and functions as an educational resource to patients, clinic staff, students, and the community. The Health Center opened as an FQHC in 2004 and is the medical home for nearly 10,000 patients. It serves a patient population that is diverse in age, ethnicity, and socio-economic status. The Health Center provides medical, dental, mental health and addiction services. Services are culturally and linguistically appropriate. CHC accepts Medicaid, Medicare, and private insurance. A sliding fee scale is available for people without insurance. Model of Care

The Health Center focuses on identifying barriers in accessing health care and reaches out to vulnerable populations. The Health Center is committed to improving health outcomes through a medical home model with a strong focus on prevention and disease management. The CHC is accredited by the Oregon Health Authority as a Patient-Centered Primary Care Home at the highest level (5 Star). The Health Center has implemented an innovative model of care that supports integration of primary care, mental health, oral health, and public health. Annual salary range: $159,488 - $188,727. This position is eligible for up to two salary raises within the first year, with potential increases at 6 months and 1 year based on performance. The following minimum qualifications are required for this position: Valid Doctor of Dental Medicine or DDS License (or ability to become licensed in Oregon at time of hire); AND At least 2 years of dental work experience required; 5 years preferred. Special requirements Valid Oregon license to administer nitrous oxide or ability to acquire. Valid Oregon driver's license. Hiring is contingent upon the successful completion of a background check.
