JOB SUMMARY Manages clinical activities, priorities, and problems, pharmacist staffing and workloads. Provides status report to pharmacy management effectively communicating issues, concerns, and suggestions as appropriate. Manages and develops clinical initiatives and determines therapeutic alternatives when drug shortages are identified and communicates with medical staff leadership and key stakeholders. Assists with budget preparation and daily financial activity specifically in relation to high cost drugs. Assists the Director and Assistant Vice President of Pharmacy with meeting administrative goals and objectives.

This position is part of the Pharmacy Department and functions as the pharmacy expert on the Infectious Diseases team, with a primary focus on supporting the OPAT program. This new role will help expand OPAT services to include transplant ID patients and develop the CoPAT service to optimize care for individuals receiving complex IV and oral antimicrobial therapies. While the main emphasis will be on OPAT and ambulatory stewardship, there will also be opportunities to participate in inpatient ID consult rounds and to precept pharmacy residents and/or students, offering a well-rounded clinical experience.

EXPERIENCE AND EDUCATION Required

Education Both PGY1 and PGY2 from an ASHP-accredited residency program; PGY1 with 5 years of progressive clinical practice experience; Bachelor's Degree of Science or PharmD from an ACPE-accredited school of pharmacy; or Doctor of Pharmacy (PharmD) from an ACPE-accredited school of pharmacy

Experience 5 years progressive clinical practice experience within specialty with PGY1 from an ASHP-accredited residency program or 8 years progressive clinical practice experience within specialty. Computer skills, data entry experience. English (speaking, reading, writing). Good general health and stress coping ability. Moderate physical effort: prolonged standing/walking, occasional lifting/carrying (up to 25 lbs.), reaching, bending, crouching, etc.

Licenses and Certifications (BLS) Basic Life Support certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) Upon Hire and (TSBP) Registered Pharmacy Technician eligible for licensure by Texas State Board of Pharmacy (TSBP) upon hire and Texas license with TSBP within 30 days; (ACLS) Advanced Cardiac Life Support required for inpatient positions prior to or within 6 months of employment. JOB DUTIES

Performs job responsibilities in accordance with organizational and departmental policies and procedures, performance improvement objectives and environmental/infection control standards. Develops/implements processes to meet/regulatory requirements.

Coordinates staff training, competencies, certification and continued education specific to specialty. Assists clinical staff pharmacists with therapeutic monitoring, patient assessments, interpretation of lab values, TPN assessment and pharmacokinetic calculations. Identifies opportunities for process improvements, develops and implements policy and procedures for prompt, safe and efficient patient care.

Actively promotes pharmacy. Coordinates and participates in multidisciplinary in-services/meetings, publishes articles, individualized drug information, rounds, consults, etc. Maintains high visibility/availability through participation in in-services, meetings and daily activities. Participates in the education/experiential training of pharmacy students and pharmacy residents.

Actively manage P&T initiatives through monitoring of documented interventions. Verifies data, develops measures and reports outcomes (Performance Improvement). Supports cost savings initiatives. Has active role in assessment and selection of cost-effective pharmaceuticals. Reviews formulary addition requests and develops monographs for presentation. Manage the formulary system and procedures for acquisition of non-formulary product.

Mentors clinical staff pharmacists and supportive personnel. Delegates appropriate/reasonable tasks and evaluates performance. Represents Pharmacy Department at P&T Committee and Service Line Meetings.

Performs computer order entry in an accurate, thorough and timely manner. Reviews/updates medication profiles (therapies, allergies, height, weight, etc.) and is cognizant of patient charges. Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seeks clarification if necessary and documents interventions (e.g., ADRs).

Actively enhances personal growth and development by participating in educational programs, literature searches, in-services and meetings. Actively participates in new employee orientation and training and evaluations.

Duts may include one or more core functions: direct patient care; interaction with human-subjects research participants; maintaining patient records; or related activities.
