Overview
The Physician Assistant will provide direct patient care and work as a member of an expanded healthcare team (social worker, dietitian, clinical pharmacist, behavioral health clinician, and a care manager) to oversee and coordinate medical care for a population of Veterans in a group practice setting. This position is onsite at the Idaho Falls VA Clinic, ID.
Responsibilities
Provide comprehensive primary care services to a defined panel of patients, including history taking, physical examination, ordering laboratories and diagnostic studies, and diagnosis and formulation of treatment plans for acute and chronic conditions within the scope of Medicine practice.
Assign responsibility for a panel of patients at a specific location and coordinate care with a multidisciplinary team.
Perform clinical examination, diagnosis, and treatment of acute and chronic diseases, preventive care, and coordination of care for a primary care patient panel.
Document patient care in clinical records, refer to specialists when input outside of PA scope is required, and ensure timely documentation and resolution of clinical alerts.
Monitor PACT team performance to assess adherence to VA standards, ensure quality and effectiveness of care, and meet VA performance measures.
Provide outstanding customer service and lead the team in assisting patient advocates and administration in resolving patient concerns.
Exercise medical decision-making and provide a broad range of diagnostic and therapeutic services for a specific group of patients.
Collaborate with staff and community and elicit cooperation from others; be a member of a larger multidisciplinary treatment team including social work, pharmacy, mental health, nutrition, and other services as necessary.
Perform other duties as assigned. Work Schedule: Monday through Friday 8:00am to 4:30pm.
