Job Description Acclaim Multispecialty Group, a partner with JPS Health Network, is seeking a motivated and ambitious advanced practice provider to join our General Family Medicine team. As the county hospital system, JPS has served the residents of Tarrant County for over 100 years. This is an opportunity for innovative and passionate individuals to treat complex medical issues for patients who need it most.
JPS is an urban teaching hospital with a long history of training physicians and medical students and offers 18 residency and fellowship programs. The hospital is located in Fort Worth’s Magnolia Village and Medical District, a vibrant area with cultural attractions and a strong community presence.
Job Summary
Outpatient Clinic
Patient panel – Primary Care – Adult and Pediatric
Responsibilities
Coordinates the multidisciplinary care delivered to their patient panel to optimize care.
Reviews patient medical records to diagnose, treat, provide care management and education to patients and families.
Makes appropriate patient referrals.
Monitors national health guidelines and those established by JPS Health Network and Acclaim for patient care management.
Provides evidence-based and compassionate care to all patients to improve outcomes and satisfaction; demonstrates patient-centered and patient-valued behavior.
Maintains a collaborative environment with referring physicians, residents, medical students, advanced practice professionals, staff, and support staff.
Supports academic, educational and research endeavors.
Attends Performance Improvement Programs and Peer Review meetings; actively participates in the process.
Partners and communicates to build a positive brand and patient/customer experience for JPS Health Network and Acclaim Multispecialty Group, including staff, vendors, partners, patients, stakeholders, and community leaders.
Serves as a resource to the community.
Performs other job-related duties as assigned.
Schedule 3x12 shifts — Monday, Thursday, Friday OR Wednesday/ALT Saturday, Thursday, Friday.
Knowledge, Skills and Abilities
Comfortable working in a large public/county facility.
Working knowledge in the assigned area and competency in assessment, diagnosis, and treatment processes relevant to the specialty.
Strong interpersonal, verbal, and written communication skills to work effectively with health care professionals and community resources.
Self-directed with initiative to develop and participate in community activities and medical outreach programs related to the specialty.

Qualifications Required Education and Experience:
Graduate of an accredited Nurse Practitioner program with an active Texas License
Current Texas APRN license with ACNP certification OR graduate of an accredited Physician Assistant Program with an active Texas License
Required Licensure/Certification/Specialized Training:
Current, unrestricted medical license to practice in the State of Texas
Unrestricted work authorization to work in the United States
