Overview

Position Overview: As a Case Manager II in an in-custody (jails) program, this role involves providing comprehensive support services to incarcerated individuals. You will assess participants' needs, develop individualized service plans, and connect them with appropriate resources to help regain stability and move toward permanent housing. The primary objective is to empower clients to overcome barriers and achieve self-sufficiency. Responsibilities

Client Assessment: Conduct comprehensive assessments of individuals experiencing homelessness to identify their needs, strengths, and goals. Case Planning: Develop individualized case plans in collaboration with clients, addressing immediate and long-term needs such as housing, employment, healthcare, and social support. Referrals and Resources: Connect clients with resources and services—including housing programs, mental health services, substance abuse treatment, job training, and legal assistance. Advocacy: Advocate for clients\' rights and needs within the shelter and the community, liaising with social service agencies, government entities, and community organizations. Crisis Intervention: Provide immediate support and intervention during crisis situations (e.g., mental health crises, substance abuse relapses, domestic violence incidents, medical emergencies). Case Monitoring and Documentation: Regularly monitor clients\' progress, document interactions, and maintain accurate and confidential case records; update case plans as needs change. Supportive Counseling: Offer emotional support, guidance, and counseling to help clients develop coping skills, problem-solving abilities, and positive relationships. Life Skills Training: Facilitate workshops and group sessions on life skills such as budgeting, job readiness, communication skills, and conflict resolution. Collaboration: Work with other case managers, shelter staff, and external service providers to ensure coordinated and holistic care. Program Evaluation: Assist in evaluating the effectiveness of the shelter\'s programs and services, providing feedback and recommendations for improvement. Professional Development: Stay updated on best practices in case management, homelessness, and related fields through ongoing training and development opportunities. Requirements

Minimum Requirements and Experience

Experience: Prior experience in a case management role is not always required for an entry-level position; any experience with vulnerable populations, volunteering at homeless shelters or community organizations, or internships/practicums in social services is advantageous. Demonstrating commitment to social justice and support for those experiencing homelessness is valuable. Knowledge: Basic knowledge of homelessness issues, social services, and local resources; familiarity with trauma-informed care, cultural competency, and client-centered approaches; knowledge of relevant laws, regulations, and policies is advantageous. Skills: Excellent interpersonal and communication skills; strong organizational and time-management abilities; empathy and compassion; problem-solving; cultural sensitivity and ability to work with diverse backgrounds. Education: Bachelor\'s Degree in a related field or a minimum of 4 years direct experience in case management. Preferred Qualifications

Commitment to professional development and enhanced candidacy. Familiarity with Harm Reduction, Motivational Interviewing, and Housing First approaches to care for the homeless population. Electronic data collection, input, and information maintenance. Excellent communication and interpersonal skills; proven ability to collaborate with other agencies. Understanding of issues faced by disenfranchised populations. Availability to work occasional evenings and weekends. Clinical supervision toward licensure is available for those on a BBS licensure track if selected. Must be computer literate and have excellent verbal communication skills. Commitment to the mission of Bayview Hunters Point Foundation and sensitivity to individuals with substance abuse and/or mental health issues (e.g., PTSD, psychosis, borderline behaviors, bipolar). Designing, planning, and implementing program services with accountability for participants as they move forward. Ensure health and safety of participants and effective management of the facility\'s physical plant, including safety, hygiene, appearance, and overall aesthetic. Prepared to respond to emergencies with clear thinking and swift action. Certified Case Manager (CCM), Certified Alcohol and Drug Counselor (CADC), or Mental Health First Aid certifications may be preferred. Bachelor\'s degree in social services, psychology, or a related field. Physical & Additional Requirements

Regular and reliable attendance and effective verbal and written communication skills. Proficiency in computer skills (Microsoft Office, Avatar, internet browsers, etc.). Respect for others to maintain professional relationships; independent judgment in evaluating options to make sound decisions. Ability to work in an open office environment with moderate noise and distractions; ability to sit, with occasional walking/standing; travel between sites as needed. Specific vision abilities; ability to meet deadlines with time constraints; occasional irregular or extended hours may be required. Valid California driver\'s license if duties require; Live Scan fingerprinting and TB clearance; other vaccinations may be required. At least two COVID-19 vaccinations (preferred). Background check clearance as part of our employment process in accordance with applicable laws and regulations.
