Boston Senior Home Care is a private, nonprofit organization that provides case management, person-centered care planning, home care and long-term services and supports to those who wish to remain in the community but require assistance to do so. One of 26 Aging Services Access Points (ASAPs) in Massachusetts, Boston Senior Home Care is committed to providing the best assistance possible for those in need. Although the majority of the people we assist are over age 60, we offer long term services and supports for children and young adults. We also provide programs and education initiatives for those serving as caregivers and information about maintaining a healthy lifestyle and controlling chronic disease at any age. We are proud of our organization, our staff, and the people we serve. We have been a part of the Boston community for more than 40 years and we love what we do. Job Summary
Case Managers conduct home visits in the Greater Boston area to assess and evaluate elder's eligibility for the State Home Care Program as well as other community resources. Following the development and implementation of the client\'s initial care plan, Case Managers monitor their assigned client caseload through home visits and telephone contact. Case Managers work collaboratively with informal and formal supports, vendors and other community agencies. We are located in Downtown Boston right off the South Station. Conduct home visits in the Greater Boston area to assess elder eligibility for the State Home Care Program and other community resources. Develop and implement the client's initial care plan and monitor their caseload through home visits and telephone contact. Collaborate with informal and formal supports, vendors and other community agencies to coordinate services. Ideal Candidate / Qualifications
Bachelor\'s degree in social work, human services, nursing, psychology, sociology, or related field is preferred, but not required. SW, LSW and relevant work experience preferred, but not required. Sensitivity to elder issues and commitment to maintaining clients at home with dignity. Precise record keeping, excellent oral and written communication skills and a commitment to teamwork. Excellent assessment and documentation skills required. Bilingual in Haitian Creole/Spanish/Cantonese/Mandarin is an added bonus. Work Authorization / Security Clearance
