Position Summary
Under the direction of the Clinical Manager, ensures optimum patient outcomes related to medication use by preventing or resolving drug-related issues and optimizing the drug therapy of patients. Provides drug information to patients and families, physicians, nurses, and other healthcare professionals. Integrally involved in education of pharmacists, residents and students. Serves as a resource and drug expert in the gastrointestinal specialty. Strives to increase knowledge through research and writing activities. The Clinical Specialist may also be involved, as necessary, in coordinating the acquisition, distribution and control of drug therapy for patients under the care of the pharmacist.

Essential Responsibilities / Duties
Responsible for pharmaceutical care within the gastroenterology ambulatory care practice setting.
Be a resource to oversee quality improvement projects, formulary decisions and other workflow designs.
Develop a practice within the various ambulatory practice sites and liaison as appropriate with other partners. Such practice should include a collaborative practice agreement in which the pharmacist, in accordance with federal, state and local statutes and regulations, acts as an independent practitioner and may evaluate and, if necessary, make appropriate treatment modifications.
Evaluate, monitor and modify drug therapy; provide recommendations to optimize patient outcomes; provide patients with new medication education; minimize safety issues; ensure cost effectiveness; and provide drug information for medical personnel.
Collaborate with stakeholders to ensure accreditation and/or statutory requirements are met (e.g., URAC, Joint Commission).
Evaluate, monitor and modify drug therapy and, if applicable, prescribe or discontinue treatments when necessary under the collaborative practice agreement.
Provide patients with new medication education.
Coordinate pharmacotherapy activities and educational opportunities for department pharmacists and residents in the ambulatory practice setting.
Participate in medication formulary review.
Assist with collection and presentation of patient outcomes data.
Provide clinical support in gastroenterology and other specialty disease priorities as needed, with focus on liver disease and treatment.
Education / Qualifications
Work requires at a minimum a Bachelor's Degree in Pharmacy from an accredited college of pharmacy and at least five years of pertinent work experience; or a Bachelor's Degree in Pharmacy with post-graduate training and at least three years of pertinent work experience; or a Doctorate Degree in Pharmacy with post-graduate training or at least two years of pertinent work experience.
Certificates, Licenses, Registrations Required
Licensure to practice pharmacy in the state of Massachusetts by the hire date.
Eligible for credentialing as a licensed independent provider practicing under a collaborative drug therapy management agreement.
