What You'll Do
Evaluate, diagnose, and treat a broad range of adult medical conditions, including chronic illnesses such as diabetes, hypertension, COPD, and heart disease. Conduct routine and preventive care visits, screenings, and wellness checks to promote long-term health and early detection of disease. Order, interpret, and follow up on diagnostic tests and lab work. Develop evidence-based, personalized care plans in collaboration with patients and their families. Manage a continuity panel of adult patients, ensuring appropriate follow-up and long-term care. Document patient encounters, diagnoses, and treatment plans accurately and promptly in the electronic health record (EHR) system. Coordinate care with specialists, behavioral health professionals, and other providers as needed. Participate in quality improvement efforts and initiatives tied to clinical performance metrics and value-based care goals. Deliver culturally sensitive and linguistically appropriate care tailored to Houston’s diverse community.
Qualifications
Completion of an accredited Internal Medicine residency program Board Certified or Board Eligible in Internal Medicine Unrestricted and active Texas Medical License (M.D. or D.O.) Current DEA registration (Texas) Current BLS and ACLS certifications Proficiency in Spanish or another second language is a strong plus Experience in a value-based care or community clinic setting is preferred Excellent clinical judgment, communication, and teamwork skills Passion for delivering high-quality, patient-centered care to underserved populations
Environmental Job Requirements and Working Conditions
This position is located in-office. The clinic address is 509 W. Tidwell, Suite 200, Houston, TX 77091 The target base salary range for this role is: $180,000 - $220,000. This salary range represents our national target range for this role Sign-on bonus and annual performance bonus structure aligned with value-based care outcomes, including quality metrics, patient engagement, and panel management performance
