Join the Street Health Team, a multidisciplinary group providing compassionate, community-based care to people experiencing homelessness in San Francisco. The team includes Behavioral Health Clinicians, Nurses, Health Workers, Physicians, and Psychiatrists, collaborating to address complex health and social needs. The Street Health Team works within assigned neighborhoods, meeting individuals where they are to provide essential services and support that promote well-being. We collaborate with city agencies to connect people with shelter, treatment, and long-term support. This is an opportunity for professionals passionate about serving San Francisco’s most vulnerable populations and contributing to the City’s approach to reducing homelessness and promoting long-term stability and health. Essential Duties

Interviews applicants, recipients, parents and others to determine eligibility for psychiatric care and services; evaluates information and factors relevant to the applicant and their situation. Decides on dispositions such as emergency therapy or referrals, completes intake forms and documents. Conducts interviews with parents and children to obtain information; determines need for testing and disposition of cases. Confers with referring agencies, schools, public health nurses and doctors regarding evaluation and treatment; prepares related reports. Follows up on individual patient therapy; maintains positive patient relationships; prepares therapy notes and integrates into case records. Maintains patient activity records, prepares clinical statistics and periodic reports; participates in staff meetings and policy development. Performs related duties as required. Minimum Qualifications

Possession of a valid license issued by the California Board of Behavioral Sciences (BBS) for either: Licensed Clinical Social Worker (LCSW) or Associate Clinical Social Worker (ASW) Licensed Marriage and Family Therapist (LMFT) or Associate Marriage and Family Therapist (AMFT) Licensed Professional Clinical Counselor (LPCC) or Associate Professional Clinical Counselor (APCC) Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted.
