Homebase Medical is expanding rapidly into the Burbank, CA area. We are currently searching for experienced part-time

Advanced Practice Clinicians (Physician Assistant or Nurse Practitioner)

to join our expanding house call practice. Key Duties

Conduct

Annual Wellness Visits

focused on patient education

Deliver

Chronic Care Management

to improve long-term outcomes

Perform comprehensive in-home visits including: Medication review & chronic illness management

Medical history updates & physical exams

Labs, diagnostic screenings, and post-hospital follow-ups

Primary care management & palliative care services

Qualifications

Active

Nurse Practitioner

or

Physician Assistant

license in the State of California

Board certification (Nurse Practitioner or Physician Assistant)

DEA licensure

Prior experience preferred (but passion and commitment are just as important)
