Assumes primary case management responsibility for a patient/family caseload, including initial assessment, continued assessment, planning, implementing, and evaluating all phases of nursing care. Assess patient needs and admissions for service when needed, and follow protocol to admit patients who meet eligibility criteria. Re-evaluate the patient’s clinical needs at least every 14 days and document continual eligibility with appropriate subjective and objective criteria. Initiate, implement, coordinate, and document the patient’s plan of care with the interdisciplinary team in accordance with nursing needs specified in the approved plan of care. Deliver or supervise skilled nursing services using evidence-based knowledge regarding end-of-life care. Document all clinical nursing visits, medication additions and changes, care plans, and other documentation per protocol within 24 hours of service. Job Requirements

Bachelor’s degree preferred. State issued RN license required. At least two years of administrative experience, preferably hospice or discharge planning. Ability to discuss end-of-life issues clearly and respond compassionately to clients under duress. Proficient in Word, Excel and able to use documentation software competently to assist others. Compensation and Benefits
