As a Clinical Pharmacy Specialist, you'll be at the forefront of patient care, leading impactful interactions that challenge traditional pharmacy roles. Utilize advanced healthcare technology to deliver direct care for chronic conditions Participate in innovative patient visits, including transitions of care, education classes, and shared visits Work in a tech-forward, agile environment focused on continuous improvement and community impact Drive growth through digital transformation and pioneering medication management approaches Key Responsibilities
Provide pharmaceutical care through direct patient and provider interactions Manage medications for chronic diseases using virtual care tools Gather comprehensive medical and medication histories for safe, effective care Perform medication reconciliation during outpatient visits Conduct physical assessments and triage patients as needed Document interactions and interventions accurately in electronic records Develop therapeutic plans focusing on efficacy, safety, and cost-effectiveness Adjust medication therapies within approved protocols Order and interpret labs/tests to optimize therapy Report adverse drug events and allergies Ensure regulatory compliance while practicing innovative pharmacy Provide drug information and counseling on interactions and side effects Support medication adherence during counseling Lead in setting priorities and collaborating across teams Mentor pharmacy students and residents Minimum Qualifications
PharmD or B.S. in Pharmacy from an accredited institution Completion of an ASHP-accredited PGY-1 Residency Valid Texas Pharmacist license CPR/BLS certification Board-Certified Pharmacotherapy (BCPS) or Ambulatory Care (BCACP) At least 2 years of community pharmacy experience Experience in medication adherence counseling Specialized experience in HIV/AIDS, Geriatrics, or Pediatrics Completion of a relevant PGY-2 Residency Immunization or smoking cessation certification Proficiency with Windows, Word, Excel Strong digital project management skills Excellent communication and customer service skills About Legacy Community Health
