Overview
Population Health Clinical Pharmacy Specialist role within Boston Medical Center Health System (BMCHS). This position supports a new cardiometabolic health initiative in the Mattapan neighborhood, working as part of an integrated, multidisciplinary team to address diabetes, hypertension, obesity, and related conditions. The CPS will collaborate with clinicians to resolve drug-related issues, optimize therapy, and support medication adherence, under the clinical supervision of the program’s medical director. Essential Responsibilities / Duties
Work in a collaborative fashion with all specialty area nurses, fellows, and attendings toward optimizing patient outcomes, with focus on improving population health activities aligned with institutional goals and mission. Provide primary care-focused pharmacotherapy services within the clinic and evaluate/manage patients with chronic conditions (e.g., diabetes, hyperlipidemia, hypertension, COPD/asthma, heart failure, smoking cessation) through direct patient care as outlined in the Collaborative Practice Agreement. Collaborate with a multidisciplinary team of primary care physicians, medical residents, nurses, and pharmacy liaisons to promote rational, safe, evidence-based, and cost-effective therapy. Identify patient barriers and develop individualized, patient-centered pharmacotherapy care plans. Maintain professional competency and participate in quality improvement activities; comply with accreditation, legal, and regulatory requirements; maintain HIPAA standards. Precept pharmacy residents and students and participate in the orientation and cross-training of pharmacists. Develop and maintain order sets and operational/clinical efficiencies in the EMR at the clinic site. Meet hospital-wide standards and follow hospital infection control and safety procedures. Set annual goals based on the institutional scorecard and under the guidance of the Senior Clinical Pharmacy Manager; identify opportunities for new care management approaches. Provide clinical coverage within the scope of license when Nurse Practitioner is not available and participate in program huddles with the clinical team. The Primary Responsibilities Specific To The Clinic Site
Monitor patients for appropriateness and response to therapy; document pertinent findings in the electronic medical record. Develop, document, and execute a patient-centered plan of care using national guidelines or established local protocol; provide ongoing care for chronic problems as delineated in protocol. Order laboratory studies and analyze data to modify drug therapy and dosing as necessary. Identify and take corrective action for drug-induced problems; notify primary provider regarding atypical findings. Provide transition of care support including medication reconciliation and post-discharge follow-up calls; provide patient education on medication regimens and build self-management skills. Deliver in-service education to nurses, physicians, and other practitioners as needed. Align pharmaceutical care with population health objectives and maintain a patient-centered approach to care. Utilize motivational interviewing to promote behavior change and collaborate with the care team to develop patient-centered care plans focused on adherence. Support program goals through patient and caregiver engagement. The Primary Responsibilities Specific To The Mattapan Program
Act as a central team member to assess and address medication-related needs for patients with cardiometabolic conditions using patient-centered care to improve self-management and outcomes. Respond to referrals from the program team and proactively reach out to patients needing clinical pharmacy services for diabetes, hypertension, and obesity. Lead rapid initiation, titration, and optimization of guideline-directed pharmacotherapy for diabetes, hypertension, and obesity; design patient-centered pharmacotherapy plans that balance adherence and chronic disease management; partner with other core team members. Work in both telemedicine and in-person settings to meet program and patient needs as it evolves. Adhere to evidence-based principles and facilitate group didactic sessions for program patients in partnership with the Nurse Practitioner. Provide clinical coverage within the scope of license when the Nurse Practitioner is unavailable; participate in program huddles with the clinical team. Education Degree in Pharmacy from an accredited college of pharmacy. Certificates, Licenses, Registrations Required: Massachusetts pharmacist license in good standing by hire date. Required: Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP); or must obtain certification within one year of eligibility. Experience Requires completion of a PGY1 pharmacy residency program; ambulatory-focus preferred. Preferred experience: Direct patient care and clinical management of primary care-based chronic diseases. Knowledge And Skills
Comprehensive understanding of procedures, systems, and equipment used by pharmacists and technicians in the pharmacy; high proficiency in performing pharmacist duties.
