Family Medicine Physician & Preceptor/Teaching position. Position Summary Provide the full scope of services within your training field, including diagnosis, treatment, care coordination, preventive care, and electronic health record (EHR) management, adhering to quality, risk, infection control, customer service, and safety guidelines of Access Community Health Network. Competitive base salary $30,000 Sign-On Bonus Productivity and Quality Bonuses $10,000 Relocation Assistance

Loan repayment options via National Health Service Corps (NHSC)

NHSC LRP: Up to $75,000 annually for 2 years. Additional incentives for Spanish-language proficiency and SUD-W programs.

Clinical and Operational Responsibilities General Clinical Obtain health histories and perform physical exams. Diagnose, treat, and coordinate care for patients. Maintain documentation standards and complete progress notes within 24 hours. Respond to on-call duties and respond to messages within 72 hours. Prescribe medications per Illinois regulations and guidelines. Follow quality standards and clinical guidelines of ACCESS. Use certified EHR technology to meet Meaningful Use standards, ensuring privacy and security. Patient Support and Education Provide culturally competent care and involve families in care plans. Encourage patient questions and self-management. Conduct educational activities and document instructions, tailoring communication to patient understanding. Coordinate with care teams and ensure appropriate transfer of care for higher acuity cases. PCMH Requirements Coordinate with health center staff to meet patient needs. Participate in health center huddles, meet productivity standards, and ensure timely care. Provider Development Participate in staff training and certification. Mentor trainees and students as a preceptor. Adhere to HRSA and FTCA regulations and perform other duties as assigned. Requirements/Preferences MD or DO in Family Medicine, with completed residency and board certification. At least 2 years of outpatient primary care experience preferred. Current Illinois licensure, DEA, and CME compliance. Knowledge of quality, safety, infection control standards; EPIC experience preferred. Note: Vaccination against communicable diseases may be required unless exemptions are granted.
