Provide the full scope of services which fall under provider’s field of training, including diagnosis, treatment, coordination of care, preventive care, and electronic health record (EHR) maintenance, while following guidelines within quality management, risk management, infection control, customer service, and safety programs for Access Community Health Network. Compensation Package Productivity and Quality Bonuses Relocation Assistance Loan repayment potential through National Health Service Corps (NHSC) NHSC LRP: Up to $75,000 Full Time for 2 years NHSC SUD-W LRP: Up to $75,000 Full Time for 3 years NHSC LRP or SUD-W LRP: $5,000 award enhancement for clinicians who demonstrate Spanish-language proficiency HPSA Primary Care score 19 for ACCESS Health Centers ACCESS Westside, Madison, and Sinai Health Centers have a HPSA Primary Care score of 21 232 Hours of Paid Time Off (PTO) per year & Paid Holidays 40 Hours of CME & $2,000 Reimbursement Malpractice liability provided by the Federal Torts Claims Act (FTCA) Flexible schedules and more… Clinical and Operational Duties Obtain health history and perform physical examinations Diagnose, treat, counsel, and coordinate care for each patient Review and update problem list and longitudinal plan of care at every visit Meet documentation standards and complete all progress notes within 24 hours following the patient visit Fulfills on-call responsibilities as scheduled and responds to calls within 20 minutes Prescribe medication according to Illinois statutes and professional guidelines Perform all duties in conformity with established quality standards and clinical guidelines Meet Meaningful Use standards by using certified EHR technology to improve quality, safety, efficiency, and reduce health disparities, while maintaining privacy and security Manage EHR In-Basket, respond to messages within 72 hours, and transfer responsibilities when appropriate Ensure all clinical activities meet reimbursement guidelines Provide culturally competent care and recognize patients’ families as part of the care team Ensure patient and family needs are met and clinic policies followed Encourage patient and family questions and involvement in care Conduct patient education and document all instructions, tailoring presentation to patient understanding Coordinate with care team and case management for comprehensive service delivery Transfer patients appropriately for higher acuity cases Meet severity of illness and admission criteria as needed PCMH Requirements Coordinate with health center staff to meet patient needs Participate in health center huddles and meet productivity standards Ensure timely and appropriate patient care involving all team members Carry out effective treatment and education per policies Provider Development Participate in staff orientation and training Ensure care team meets internal certification standards Participate in proficiency testing to maintain care standards May serve as a preceptor for training programs Adhere to HRSA and FTCA regulations Other duties as assigned Requirements/Preferences Current PA license in Illinois and CME requirements Board Certification required Obtain and maintain a collaborative agreement and DEA license Two years outpatient primary care experience preferred Knowledge of quality assurance, safety, and infection control standards EPIC EHR competency preferred Cultural competency and interpersonal skills essential Strong organizational skills required Working Conditions/Equipment Ability to concentrate and communicate in busy environment Stand, sit, and walk for up to eight hours Use of computer/laptop ACCESS is a network of Federally Qualified Health Centers. Depending on the position, vaccination against communicable diseases may be required, with exemptions available for religious or medical reasons. Compensation varies based on experience, education, market conditions, and funding.
