Overview The Psychiatric Nurse Practitioner works on a multi-disciplinary team supporting Psychiatry services imbedded within primary care sites across counties (Napa, Solano and Yolo) and may include virtual visits to minimize travel.

Minimum Position Requirements: Education, Experience, Skills/Training

Education: Graduate of an accredited Master of Science in Nursing required; 2 years minimum working in the behavioral health/mental health field.

Experience/Lived Experience: Experience in a community healthcare center preferred; bilingual (English/Spanish) language preferred; must be able to adapt and multi-task to manage a high volume of patients.

Special Skills/Training/Requirements: Current California Nurse Practitioner certification required; Current Board certification as Psychiatric Mental Health Nurse Practitioner preferred; Current California medication furnishing license required; Current DEA License required; Current Basic Life Support CPR certification required.

Essential Functions and Responsibilities

Knowledge of and implementation of crisis interventions, knowledge of applicable laws and ethical consideration related to patient rights, treatment, safety and confidentiality.

Knowledge of the current Diagnostic Statistical Manual of Mental Disorders and ability to accurately use this knowledge to diagnose patients and inform treatment planning.

Knowledge of Federally Qualified Health Centers and the key health center principles and guidelines.

Provides comprehensive psychiatric assessment, diagnosis, prescription of drug therapy, planning of treatment, and evaluation of therapy.

Provides brief therapy-based interventions as needed.

Prescribes psychotropic medications based on clinical indicators and with consideration of chronic diseases. Review medication(s) that patient is currently taking, prior to prescribing to prevent adverse reactions.

Provide patient education and advice regarding side effects and dosage adjustments. Monitor/evaluate pharmacological treatment progress.

Orders appropriate lab tests and interpret results. Review and manage patient information including labs, prescription refills, outside consultation notes, previous medical records, specialist referrals, etc.

Provides feedback and consultation to primary care medical providers and other treatment or service providers in the community regarding the behavioral health and/or psychiatric aspects of the patient. Offer specific recommendations related to integrated treatment planning.

Prioritizes referrals and schedule appointments, providing crisis intervention and follow ups as needed.

Refers patients to relevant patient care services in the community as appropriate. Advocate on behalf of patients to obtain services as necessary.

Provides education and support to patients and their families to encourage compliance with medical and behavioral regimes.

Documents all appropriate information in the electronic health record system.

Responds to messages and relay information to patients and other external sources as needed. Write letters documenting and/or summarizing diagnosis and treatment as requested and appropriate.

Submits Treatment Authorization Requests to insurance companies as appropriate. Complete various other medical related forms as needed.

Assists in the planning and facilitation of educating providers on behavioral health topics.

May participate in and collaborate with staff/teams on various support-related functions including quality assurance, care team development, system optimization, empanelment, policy and procedure development, etc.

Works with Behavioral Health Manager to facilitate administrative aspects of the program.

Adheres to attendance and punctuality requirements per health center policy. Provides proper notification for absences and tardiness. Takes corrective action to prevent recurring absences and tardiness.

Observes all health center and departmental policies governing conduct while at work (e.g., telephone and computer use, electronic messaging, smoking regulations, parking, breaks and other related policies).
