0.8 to 1.0 FTE (32 to 40 hours/week) DLSE/FLSA STATUS: ( ) Exempt /Salaried position or Nonexempt/Hourly position SUPERVISORY RESPONSIBILITIES: YES or NO About CommuniCare+OLE Established in 2023, CommuniCare+OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs. Full Time Benefits

The Psychiatric Nurse Practitioner works on a multi-disciplinary team supporting Psychiatry services embedded within primary care sites across counties (Napa, Solano and Yolo) and may include virtual visits to minimize travel. Education: Graduate of an accredited Master of Science in Nursing required 2 years minimum working in the behavioral health/mental health field Experience/Lived Experience: Experience in a community healthcare center preferred Bilingual (English/Spanish) language preferred Must be able to adapt and multi-task to manage a high volume of patients Current California Nurse Practitioner certification required Current Board certification as Psychiatric Mental Health Nurse Practitioner preferred Current California medication furnishing license required Current DEA License required Current Basic Life Support CPR certification required Essential Functions And Responsibilities
Provide comprehensive psychiatric assessment, diagnosis, prescription of drug therapy, planning of treatment, and evaluation of therapy Prescribe psychotropic medications based on clinical indicators and with consideration of chronic diseases; review current medications to prevent adverse reactions Provide patient education and monitor pharmacological treatment progress Order appropriate labs and interpret results; manage patient information including labs, prescriptions, notes, referrals, and records Collaborate with primary care providers and other treatment or service providers regarding behavioral health and psychiatric aspects of care Prioritize referrals and schedule appointments; provide crisis intervention and follow-ups as needed Refer patients to community services and advocate to obtain services as necessary Educate and support patients and families to promote adherence to medical and behavioral regimens Document all information in the electronic health record system Respond to messages and relay information to patients and external sources; compose letters documenting diagnosis and treatment as appropriate Submit treatment authorization requests to insurers and complete related forms Assist in educating providers on behavioral health topics; participate in team and program development activities Support administrative aspects of the program in collaboration with Behavioral Health Manager Seniority level
