As a Home Health Physical Therapist, you will evaluate, develop, and implement individualized treatment plans for patients in their homes. Your expertise will help restore physical function, reduce pain, and maximize independence. You will work with a multidisciplinary team and provide guidance, support, and education to patients and their families. Job Responsibilities
Conduct comprehensive assessments of patients\' physical conditions and functional abilities in home settings. Develop individualized treatment plans and goals based on assessment findings and patient needs. Provide evidence-based physical therapy interventions, exercises, and modalities to promote recovery and independence. Monitor patient progress and adjust treatment plans as needed to ensure timely interventions. Educate patients and families on techniques, exercises, and self-management strategies. Collaborate with physicians, nurses, and occupational therapists to ensure coordinated and holistic patient care. Maintain accurate documentation of evaluations, treatment plans, progress notes, and outcomes. Adhere to professional standards, ethics, and regulations, maintaining confidentiality and patient rights. Qualifications
Valid and active Physical Therapist license in California. Minimum of 2 years of experience as a Physical Therapist, preferably in home health or related settings. Strong clinical skills across musculoskeletal and neurological conditions. Excellent communication and interpersonal skills to work with patients, families, and healthcare professionals. Ability to work independently and manage a caseload. Reliable transportation and willingness to travel to patients\' homes within the San Francisco area. Proficient computer skills and familiarity with electronic health records (EHR). Benefits
