The Center Medical Director leads the clinical team at their Oak Street Health center to provide the best patient care and maintain a great place to work, aiming to keep patients healthy and out of the hospital. The CMD will practice primary care in clinics at 80% clinical time and 20% administrative time, mentor and coach providers, and participate in organizational-wide strategic initiatives. Responsibilities

Serve as the medical leader for the OSH center and drive the center’s clinical performance and culture. Support organizational strategy in collaboration with the Practice Manager and Outreach Manager/Director to achieve objectives such as Best Care Anywhere, Unmatched Patient Experience, Exceptional Growth, and Great Place to Work with a focus on DE&I. Drive implementation of OSH strategic initiatives and align with regional priorities as directed by regional leadership. Ensure effective communication of organizational information to care teams and escalation of concerns to the organization. Provider Management

Coordinate onboarding of new providers at the center and model high-quality medical care and care model implementation. Support robust OSH care model execution, including daily huddles, patient care planning, admission prevention, and clinical documentation workflows. Maintain evidence-based care to keep patients healthy and out of the hospital; implement Oak Street’s clinical guidelines at provider and center levels. Support high-quality preventive care and educate providers on the value-based full-risk model. Assess and address root causes when center metric goals are not achieved. Leadership and Coaching

Foster a culture of coaching and growth among providers and care teams. Solicit feedback and address concerns with constructive problem-solving; conduct performance reviews and help define development plans for providers. Culture

Acknowledge successes and identify opportunities for improvement; promote a positive culture of joy in practice. Support adherence to best practices and other duties as assigned. Qualifications

M.D., D.O., APN or PA graduate. Board certification in Internal Medicine or Family Medicine for M.D./D.O.; Board certification in Family, Adult, or Gerontology for APN. Experience in clinical leadership, coaching providers to deliver excellent care. Experience with a metrics-driven approach to medical care and quality projects. Passion for the work and motivation to contribute to a fast-growing organization. Leadership potential for impact within a fast-growing, innovative team. Commitment to older adults and to low-income patients. Ability to handle ambiguity and solve problems calmly; ability to guide others to effective solutions. 5+ years of outpatient practice experience preferred. Fellowship in Geriatrics or additional degrees (e.g., MBA, JD, MPH) and prior executive roles welcomed but not required. US work authorization. Details

Anticipated Weekly Hours: 40 Time Type: Full time Pay Range: $174,070.00 - $374,920.00 Note: This pay range is base salary; actual offer depends on experience, education, geography, and other factors. Benefits
