Overview

The Center Medical Director role at Oak Street Health is to lead the clinical team at their center in providing the best patient care and a great place to work in order to keep patients happy, healthy and out of the hospital. The CMD will practice primary care in our clinics alongside their provider colleagues at 80% clinical time and 20% administrative time. The CMD will have mission-critical leadership responsibilities, including mentoring, coaching, and advising providers, and participating in organizational-wide strategic initiatives. Responsibilities

Lead the medical center as the medical leader; collaborate with Practice Manager and Outreach Manager/Director to achieve objectives. Drive implementation of OSH strategic initiatives and regional priorities. Support organizational strategy and cascade information; escalate care team concerns. Onboard and manage providers; model high-quality care; ensure care model implementation; supervise daily huddles, care planning, admission prevention, and documentation workflows. Ensure evidence-based care and adherence to Oak Street's clinical guidelines. Support preventative care and value-based full risk model; address root causes when metrics are not met. Lead and coach providers and care teams; conduct performance reviews and development plans. Foster a culture of coaching and growth; listen to feedback and resolve concerns. Maintain a positive culture and celebrate successes; support triad best practices. Other duties as assigned. Ideal Candidates Will Be

M.D., D.O., or APN/PA graduates. Board certification in Internal Medicine or Family Medicine for M.D./D.O.; Family, Adult, or Gerontology for APN. Experience in clinical leadership roles and in leading/coaching providers. Experience with metrics-driven medical care and quality projects. Passionate, motivated to work in a fast-growing organization. Leadership potential and collaborative mindset for a fast-growing team in healthcare innovation. Commitment to older adults and low-income patients; calm under ambiguity; problem-solving mindset. 5+ years outpatient practice experience preferred. Fellowship in Geriatrics or other advanced degrees welcomed but not required. US work authorization. Anticipated Hours

Full-time Pay Range

The typical pay range for this role is $174,070.00 - $374,920.00 per year. The actual base salary offer depends on experience, education, geography, and other factors. Benefits
