As a
Family Practice Provider in our Albany Clinic , you will work as a part of a primary health care team in collaboration with a team of Physicians and Providers. In this role, we are looking for an individual with a desire to work in a collaborative model. You will be partnered with a Physician and will share and continue to grow a patient panel. Schedule
Full-time, up to 1.0 FTE, M-F Responsibilities
Chronic disease management
Evaluating, implementing and managing patient treatment based on assessments of their needs
Reviewing lab results and planning the course of action according to the results
Providing preventative care to adults and children
Educating patients on health and wellness
Requirements
To be eligible for this position you must meet one of the following requirements; Certified as a Nurse Practitioner
Current Minnesota Physician Assistant Licensure
We are hiring an applicant that meets or exceeds the following experience: Previous experience in Urgent Care
Previous experience Family Medicine/Walk-In Clinic
Pay and Benefits
Starting pay begins at $109,241.60 per year; exact wage determined by years of related experience
Salary range: $109,241.60 - $124,904 per year
Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
