Status: Full-Time
Location: Rye, NY (The Osborn campus)
Schedule: Monday–Friday, 9:00 AM – 5:00 PM
Pay Range: $45.00–$53.99/hour (commensurate with experience)
Bring clinical excellence and heartfelt service to older adults who call The Osborn home. As a Campus RN, you’ll coordinate and deliver on-campus nursing services, ensure plans of care are accurate and up to date, and partner closely with residents, families, and our interdisciplinary team to support safe, dignified living.
What you’ll doAssess & plan: Complete nursing assessments, develop individualized plans of care, obtain/implement physician orders, and update plans as conditions change.
Coordinate care: Collaborate daily with Home Care leadership, case managers, rehab, social work, and community clinicians for smooth, consistent care.
Educate & advocate: Teach residents/families about medications, disease management, and safety; serve as liaison with outside providers.
Document with accuracy: Maintain timely, compliant documentation and support quality/performance improvement initiatives.
Support the team: Provide clinical guidance to LPNs/HHAs and reinforce standards, policies, and best practices.
License/Certs: Current NYS Registered Nurse license; BLS/CPR required. (BSN preferred.)
Experience: 2–3+ years in home care, sub-acute, or acute care (geriatric experience a plus).
Skills: Strong assessment and critical thinking, clear communication, EMR proficiency, and a calm, compassionate approach.
Mindset: Professionalism, patience, and teamwork—comfortable coordinating across disciplines and supporting resident-centered care.
Ability to move throughout campus; sit/stand/bend for extended periods.
Lift/carry 10–50 lbs; assist with resident mobility when needed (with equipment/support).
Communicate clearly in English—in person and by phone—with professionalism and discretion.
