CareNational is seeking an experienced Family Nurse Practitioner (FNP) to provide high-quality longitudinal and preventive care to families in the Lake Havasu area. This role offers a hybrid schedule, a competitive salary, and the opportunity to work with a robust interdisciplinary team.

Full-time schedule: Monday Friday, 8:00 a.m. 5:00 p.m. , plus one half-Saturday per month

Interdisciplinary team support (MDs, LCSWs, RNs, psychiatrists, dieticians, medical assistants)
Direct impact on improving family health outcomes across pediatric and adult populations
Responsibilities
Serve as the primary care provider for a family panel, delivering preventive, acute, and chronic condition management in the home and virtual settings (video, phone, SMS)
Collaborate with an interdisciplinary care team through daily huddles and weekly case conferences
Collect and document patient data, organize information, and track care plans using electronic health records and care management tools
Diagnose and treat chronic and infectious diseases; prescribe and reconcile medications; educate patients and families on conditions and treatments
Order, perform, and interpret diagnostic tests as needed
Support health goal setting and care coordination for underserved and vulnerable populations
Qualifications
Completion of an accredited Family Nurse Practitioner program
Active, unrestricted Arizona license to practice as an FNP
At least 2 years of clinical experience caring for pediatric (including newborns) and adult patients with chronic medical and behavioral health conditions
Demonstrated proficiency in clinical nursing skills (phlebotomy, patient assessments, and chronic care management for conditions such as diabetes, hypertension, CHF, depression, and respiratory concerns)
Strong independent clinical judgment and problem-solving skills
Availability for full-time work, including rotating call coverage with occasional evenings and weekends
Current CPR/BLS certification
Experience leading an interdisciplinary care team preferred
Startup environment experience is a plus, but not required
