Overview
The Wellness Nurse is responsible for monitoring the health and well-being of residents and provides leadership support and assistance with resident care and services. Responsibilities include promoting the highest quality of care in accordance with federal, state/provincial, and local standards, Sunrise Senior Living policies, and the Mission: to champion the quality of life for all seniors.
Responsibilities & Qualifications
Coordinate health needs and ensure the ongoing quality of resident services. This includes training and monitoring medication care administration, communicating with resident support services and families, and maintaining clinical quality assurance.
Essential Duties
- Identify ongoing needs and services of residents to promote high-quality resident services.
- Ensure proper follow-through, assessment, and documentation for changes in health care services and status.
- Assist Resident Care Director (RCD) with resident assessments at move-in, 30 days post move-in, semi-annually, and with any status change.
- Conduct monthly wellness visits for all residents.
- Assess, monitor, and follow up on falls, accidents, or incidents; document and report as required.
- Document pertinent information in the resident wellness file and maintain communication with families or responsible parties.
- Contact the resident's attending physician as needed or upon family request.
- Ensure weights and vital signs are obtained monthly for each resident.
- Maintain wellness files according to Sunrise policies and applicable regulations.
- Maintain medical supplies and emergency kits for the community.
- Provide clinical support and assistance to community team members as needed.
- Understand and practice infection control; promote infection control standards and resident testing as directed.
- Adhere to universal precautions for exposure to blood borne pathogens and hazardous materials.
- Medication Programs
- Monitor each resident's medication and treatment profile to ensure accurate administration and documentation.
- Transcribe physician orders to MAR/TAR with appropriate initials, dates, and times; coordinate with pharmacy as needed.
- Complete MAR/TAR audits monthly.
- Demonstrate knowledge of the Six Rights of Medication Pass (Right resident, medication, dosage, time, route, right to refuse).
- Act as liaison for pharmacy services to ensure effective services for residents.
- Monitor the Medication Room, medication carts, and treatment carts for organization and safety.
- Complete medication observations and skills checks as directed by the RCD.
- Quality Assurance and Regulatory Compliance
- Demonstrate knowledge of key quality improvement areas: Resident Centered Care Model, APIE, Quality Care Indicators and Outcomes.
- Participate in Sunrise Quality Assurance Meetings and Clinical Meetings.
- Maintain knowledge of applicable Federal, State/Provincial, and Local regulations; Resident Rights; and the Resident Assessment/Evaluation process.
- Review Physician Reports of all new residents as directed by the RCD.
- Order medications and equipment as directed by the RCD.
- Ensure documentation supports resident care programs (skin, nutrition, behavior, falls, bowel/bladder).
- Assist in implementation, training, and monitoring of documentation related to residents’ clinical health and wellness.
- Report changes, concerns, or solutions to the RCD; contribute to engagement programs and planning workshops.
- Attend regular meetings as directed and maintain required training compliance.

Experience and Qualifications
To perform this job successfully, an individual must have:
- Graduate of an approved college/school of nursing
- Current state/provincial license as a Registered Nurse (RN)
- Minimum of one year experience in home health, assisted living, hospital, or long-term care
- Knowledge of nursing practices, regulations, and assessment skills
- Proficiency in computer skills (Microsoft Office, Sunrise applications) and ability to learn new applications
- Ability to work weekends, evenings, and flexible hours
