We are seeking an experienced and highly motivated Travel Registered Nurse Case Manager to join our dynamic acute care team. The ideal candidate will be a vital part of our care coordination process, ensuring efficient, high-quality patient care from admission through discharge in a fast-paced hospital environment. This role requires a proactive professional skilled in discharge planning, utilization review, and collaborating with multidisciplinary teams.
Active California Registered Nurse (RN) License. Minimum 2 years of recent acute Hospital Case Management experience. Must have a proven background in Care Coordination and Discharge Planning within a high-volume, fast-paced acute care setting. Travel nursing experience is required. Required:
Proficiency with Epic charting system. Perform comprehensive needs assessments and develop individualized plans of care. Execute efficient and safe discharge planning, including independent arrangement of DME, SNF placements, and ambulance transport. Determine medical necessity and perform utilization review using standard criteria (InterQual experience preferred). Coordinate care effectively with patients, families, physicians, and payers (HMOs, IPAs, etc.). Ensure compliance with regulatory standards from CMS, The Joint Commission, and other governing bodies. Manage prior authorizations and have a working knowledge of DRGs and benefits eligibility. Ambulatory, Behavioral Health, Home Health, Hospice, Insurance, or LTAC/Rehab. Case Management:
Data Abstraction, Disability/Worker’s Comp case management, Disease Management, and utilizing Evidence-Based Guidelines. Experience in Behavioral Health, ER, ICU, NICU, Pediatrics, or Step-Down/PCU units. Regulatory Knowledge:
