The Integrated Care Center Clinic is a federally qualified health center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need, including those who are homeless, have mental illness or substance use disorders, and chronic medical conditions. We provide team-based care in an integrated care setting. We are looking for a mission-driven, service leader who works well in a team, has experience managing and supporting staff, champions quality improvement, leads change, and is excited to innovate. The Enhanced Case Management program under the Integrated Care Center will provide intensive, client-centered case management to support clients in achieving stability, health, and self-sufficiency. Responsibilities
The Case Manager will maintain a caseload of members served under the Managed Care Plan (MCP) by linking clients to appropriate services to address needs such as health benefits, mental health, substance use disorder, physical health, employment, housing, and community resources. Engage eligible people from State Prisons, County Jails, hospitals, and other locations for enrollment into the MCP. Use multiple engagement strategies (in-person, mail, email, texts, telephone, outreach) including home or community-based meetings when appropriate. Maintain regular contact with all providers on the member’s care team, including Justice Involved oversight. Ensure continuous, integrated care among all service providers and coordinate with primary care, physical and developmental health, mental health, SUD treatment, and housing or other community supports as needed. Support engagement in treatment by coordinating medication review/reconciliation, scheduling appointments, reminders, transportation, and addressing barriers to engagement. Engage with a multi-disciplinary care team to identify gaps in care and coordinate input from primary, behavioral, developmental, oral health, and community supports. Assist with intake by completing assessments required by the MCP. Monitor progress toward care plan goals and provide input on treatment plans. Assess client care management needs and complete all releases of information. Connect clients to benefits, healthcare services, employment, housing, community resources, and other supports. Coordinate communication and external service linkage, including scheduling appointments, communicating with probation and Family/Children Services, obtaining court minute orders, and providing appointment reminders. Collaborate with clients and families to support reintegration into the community and work with outside agencies to obtain needed resources (housing, sober living, recovery services, medical facilities, employment/education, DPSS, DMV, etc.). Clinical Documentation and Administrative Responsibilities
Write and complete all progress notes within 24 hours of service delivery. Write client progress letters and court reports. Complete all assigned peer reviews (chart audits) in a timely manner. Participate in group supervision, agency, and team meetings; engage in training opportunities. Training, Supervision and Quality Improvement
Attend and participate in case conferences to discuss treatment needs collaboratively. Participate in agency and team meetings and training opportunities. Communicate with all members of the behavioral health team; complete Relias trainings. Field-Based Responsibilities
This is a field-based position requiring frequent travel. A major component is meeting ECM members where they are located and providing case management in accessible locations, with an office available for case management activities, supervision, and required trainings as needed. Qualifications
Education, Certification, or Licensure Case Manager I: High school diploma or equivalent required. Case Manager II: Bachelor’s degree in a human services-related field (social work, public health, psychology). Minimum two years in the human services field and/or experience in substance use treatment, relapse prevention, and recovery preferred. Current First Aid and CPR certification. Valid California driver’s license and access to insured transportation. Experience with clients experiencing acute withdrawal from substances; trauma-informed services preferred. Knowledge and experience with trauma-informed services. Background Check and Other Requirements Qualified candidates with arrest and conviction records will be considered for employment. Must be able to complete a background check and Livescan. Covid-19 vaccine and booster is required for this position. Exemptions are not accepted.
